An exciting opportunity has arisen to join our friendly team, the Integrated Admiral Nurse Service as a Secondment or a 12 month Fixed Term contract to work within our Team as an Assistant Practitioner to help and support carers who look after their loved ones who have a diagnosis of dementia.

You will work alongside the Admiral Nurse Clinical Lead and three other Admiral Nurses to contribute to the support that carers and families may need in their own homes, during the hospital stay and across care transitions. You will do this through providing ongoing and compassionate support and guidance and ensuring that carers are signposted appropriately in order to help prevent carer breakdown.

This is a challenging developmental opportunity to make a significant impact on the delivery of the care of families affected by dementia across the dementia care pathway in Cornwall, working across different care settings.



This post has been developed to support the Admiral Nurse Service to provide relationship-centred dementia care to families with complex needs.

Provide a responsive, personalised information, signposting and support service to people living with a diagnosis of dementia, their carers and family in order to help them to navigate and access appropriate services in the right place at the right time.

Support the interface between mainstream general hospital services, community health and social care services and other services across the dementia pathway.

Work collaboratively with healthcare and multi-agency colleagues, supporting the work of the three Admiral Nurses and Admiral Nurse Clinical Lead across the county.

Attend conferences, dementia related events representing the Admiral Nurse Service in a professional and engaging manner.

You should have a strong base of clinical experience and you will need to have skills to contribute to clinical decision-making as part of the wider team. We are looking for an individual who can develop effective and collaborative working with families affected by dementia and healthcare and multi-agency colleagues.

This is a 37.5 hours per week post (5 days per week) and is a one-year fixed term post or secondment. (with current manager approval before applying).
