The post holder will work as part of a team for Primary Care Extended Access Service based in Ringwood. The post holder will provide clinical assessment and treatment for the delivery of a safe, efficient and reliable clinical service. They will work closely with the Clinical Lead, and Service Manager. They will form links and develop relationships with secondary care and GP Service colleagues and those working in the wider healthcare community. The post holder will ensure patients receive optimal care in the correct setting within the healthcare system. Health policies and standard operating procedures will be adhered to and, if relevant, refined. In the planned primary care clinic the patient will undergo clinical assessment and review, examination, appropriate investigations requested e.g. x-rays, scans, blood tests; and a diagnosis and management plan will be agreed. On occasions onward referrals may be needed to specialities and an expectation of a credible referral should be made. The service is staffed by a team of GPs, Nurses, Paramedics, Healthcare assistants and non clinical workforce. Service Background PHL now are working with the local healthcare community to provide the extended access Primary Care Service appointments in line with GP Improved Access hub criteria, with full access to patient medical records using EMIS. The purpose is to work collaboratively and support improving patient outcomes and experience for patients in Primary Care.
