WSG has partnered the National Council of Social Service (NCSS) and Singapore University of Social Sciences (SUSS) to offer the Career Conversion Programme for Social Workers (CCP SW) to support mid-career individuals. in acquiring knowledge and skills needed to become qualified Social Workers. Trainees can gain valuable experience while studying, and the practicum sessions in CGH will enable trainees to be effective in their respective fields upon graduation. There are two part-time courses offered under CCP SW - Bachelor of Social Work and Graduate Diploma of Social Work.

Job Description
You will help to identify, develop, be trained and perform the straightforward interviewing and assessment of patients for financial assistance. You will help in screening patients to escalate to Medical Social Workers (MSW) for intensive case management and counselling.
- Assist MSWs in social work context / patients' matters
- Interview and counsel patients and their families to help them with their psycho-social problems
- Communicate with medical staff, nurses, other health-care professionals to discuss plans for total patient management
- Liaise with other government departments, social services organisations or private agencies to meet patients' needs
- Maintain case records on patients
- Home visits to patients' homes to assess social conditions and family dynamics, counsels and gives support.
- Facilitate financial/care application process – e.g. ATF, MAF, MF, HCA, downgrading
- Counter duties; handle Department Hotline
- Any other admin duties as assigned.
Requirements
i. 24-month Bachelor of Social Work (BSWK), applicants must be non-degree holders with:
- GCE 'A' Level with two passes (prior to 2006) or two H2 Passes (from 2006); or Local Polytechnic Diploma; or NUS High School Diploma or SSI Higher Diploma in Social Service/WSQ Diploma in Social Service; or
- Any other equivalent.
ii. 18-month Graduate Diploma of Social Work (GDSWK), applicants must possess a non-social work undergraduate degree from a recognised institution.
Candidates should meet the eligibility criteria of the CCPSW and SUSS for the Bachelor in Social Work or Graduate Diploma in Social Work.
