MAIN DUTIES/RESPONSIBILITIES Clinical Responsibilities The role is to assess, plan, implement and evaluate care in association with a registered practitioner and independently. Provide and deliver a high standard of compassionate care recognising and meeting with specific needs of the patients within set protocols and within the scope of the practitioner role. Acknowledge any limitations of competence in accepting care activities outside agreed limitations and inform a registered practitioner. Seek supervision to ensure that personal clinical practice is continuing to develop. Participate in appropriate training to ensure safe, competent clinical practice Ensure that all documentation is recorded in accordance with the NMC and Trust guidance. Act at all times in the best interest of the patient. Carry out clinical observations on patients, recognising, recording and escalating any adverse signs. Undertake and perform clinical skills within specific competency boundaries e.g. venepuncture, cannulation and midline placement. Assist in the safe moving and handling of all patients utilising appropriate equipment Utilise early warning score indicators (e.g. NEWS2) to recognise any patient deterioration/sudden change in condition, escalating immediately to an appropriate practitioner Assist in safe and appropriate use and maintenance of equipment Organise and prioritise time effectively. Act in such a way that safeguards the health and wellbeing of children & vulnerable adults at all times. Is familiar with and adheres to, the PAH safeguarding policies Assist in the induction, training and assessment of learners and other staff and supporting band 2/3 HCA to practice competently with the Vascular Access Nurse Coordinate and supervise clinical support workers and other learners in care delivery Promote and participate in audit process for monitoring and reviewing quality. To be alert to issues of risk and concern about care provided raising any concerns observed via the trusts whistleblowing policy and supporting processes. Specific Roles related to area of work and within specific competency boundaries Be skilled in Aseptic Non-Touch Technique and be able to provide after-care for the lines across the trust, placed by the vascular access team or by the interventional radiology team. Recognise the need for escalation when managing the deteriorating patient in the Units, ward or departments. Ensure the environment is clean and prepared for, including stock levels for the treatment trolley Deliver effective nursing care to patients receiving PICC line insertion, midline insertion and post-insertion care under the direction of the Vascular Access CNS. Communicate effectively with patients and all members of the clinical teams to ensure information is shared in a timely and appropriate manner where appropriate Participate in the Trust Appraisal process to ensure that you receive and give constructive feedback to enhance personal skill set Take all reasonable precautions to ensure that you do not endanger yourself or others by act or omission At all times, contribute to the maintenance of the health, safety and security of individuals and their environment, reporting to the nurse in charge any hazards which may endanger individuals and where suitable take appropriate action Demonstrate a basic understanding of the reasons for blood tests and knowledge of the results and the importance of timely escalation of results. Carry out blood collection from fixed or central lines in accordance with competency and trust policies and procedures Be competent to access all forms of vascular access devices such as PICC lines, midline catheters, port-a-cath and Groshong lines. Be competent to change PICC/midline dressings in accordance to Trust Policy Safely care for individuals with long term IV access devices (e.g. PICC/porta-cath) Demonstrate an understanding and your role in the recognition of neutropenic sepsis. Other Responsibilities Contribute to the prevention and control of infection in line with trust guidelines Assist in maintenance of accurate records and documentation, with regard to legal requirements, confidentiality and management of information. Ensure the essential information on the patients condition and progress is recorded in accordance with trust guidelines. Understand the need to maintain and respect patient confidentiality Promote and value the rights, responsibilities and diversity of people ensuring religious and cultural needs are addressed. 3 COMMUNICATIONS & RELATIONSHIPS Establish and maintain effective communication with patients, significant others and patient groups. Develop and maintain appropriate communication with members of the multi-disciplinary team in all aspects of care delivered. Demonstrate a variety of communication skills with patients who may have communication difficulties e.g. confusion, dementia or other linguistic issues Ability to deal with and respond appropriately to concerns or complaints from patients or members of the public and know who to escalate the concerns to. Maintain clear, concise and legible documentation adhering to standards in accordance with trust policies and guidance. Ensure compliance with trust policies and procedures regarding confidentiality of data and data security. Communicate with colleagues and members of the multi-disciplinary team clearly and effectively in written, verbal and electronic format. 4 PHYSICAL DEMANDS OF THE JOB Frequent standing, sitting, walking kneels and crouches to attend to patient needs. Responding to changes patient condition. Moving & Handling of patients and equipment on a frequent daily basis. Frequent mental effort/concentration required in relation to responding to and meeting patients nursing care needs (e.g. for 7.5hr hour shift) able to follow direction by a registered practitioner. Verbal and written communication skills required. Exposure to difficult situations including unwell/ distressed patients and relatives/carers. 5 MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Exposure to difficult situations including distressed patients and relatives/carers. Temperature should be controlled, but may experience hot or cold work environment. Smells dealing with body fluids. Violence & Aggression may experience violence and aggression from patients and or relatives or carers. Infection control exposure to patients with infections. 6 KNOWLEDGE, TRAINING & EXPERIENCE REQUIRED TO DO THE JOB (Person Specification) Qualifications It may be necessary to complete additional modules/competencies within the first 12 months of employment to comply with trust core competencies. Experience Experience of working within an acute healthcare setting delivering clinical care. Utilisation of planning and organising skills Experience in working in interventional procedures or similar area. Training Commitment to undergo training as required Commitment to complete and pass additional clinical competencies required for the post Be involved in training others Special Knowledge Insight into role and responsibilities of post Computer literacy Current issues in healthcare Disposition/Attitude Literate and numerate Competent in written and verbal communication skills Competent in the use of information technology Able to organise and prioritise workload Able to accept delegation and work without direct supervision Practical Skills Cannulation/phlebotomy experience Patient moving and handling Competent in written and verbal technology Good manual dexterity Able to move equipment (mobile service) 9.
