Position: Administrative Assistant.

Type: Full-time, Five-day work week (Monday to Friday)

Occassionally alternate sat.

Key Responsibilities:

• Provide administrative support to ensure smooth operations within our team

• Manage documentation, filing, and data entry with accuracy

• Coordinate appointments, meetings, and office supplies as needed

• Support various departments with administrative tasks

• Assist with daily office management to maintain a productive work environment

Requirements:

• At least 3-5 years of experience in an administrative role. (In more & less, relevant industry will be a plus)

• Strong organizational skills and attention to detail

• Proficiency in Microsoft Office (Word, Excel, PowerPoint)

• Excellent communication and interpersonal skills

• A proactive approach and the ability to work independently

* Able to read or write simple Chinese.

JOB PURPOSE:

Assist with post-acute care coordination and complex disease management.

JOB RESPONSIBILITIES:

TOC Responsibilities:

• Reinstate home care hours
• DME orders
• Schedule transportation
• Enter order for the resumption of care and ensure completed.
• Enter service plan and authorizations for subacute rehab.
• Schedule CL provider post-hospitalization appointments (and CBPCP if needed).
• Schedule all specialist follow up appointments.
• Help with reminders and ensuring effectuation of these appointments, call offices for note retrieval.
• Other duties as assigned.

CCT Responsibilities

• Managing disease registries through chart reviews, apt scheduling, and communication with IDT and participant/caregiver. This may include but is not limited to the management of DM, COPD, and CHF.
• Serving as a lesion between CCT and site interdisciplinary team (IDT) to notify of and assist with addressing care gaps. This may include but is not limited to: coordinating lab draws, site visits, and referrals.
• Participating in high risk-high impact (HR-HI) panel management, provider huddles, and core site IDT reviews. Coordinating necessary follow up for HR-HI and disease registry management.
• Facilitating and coordinating TOC episode and seamless transition to HR-HI panel management for rising risk participant.
• Assisting with referral to and ensuring completion of palliative program referrals for appropriate participants
• Other duties as assigned.

Schedule: 8:30AM – 5:30PM

Weekly Hours: 40

QUALIFICATIONS:

Education: Bachelor’s degree in business, healthcare admin, or related field.

Experience:

• Two years of experience in a healthcare setting preferable.
• Experience with care coordination, discharge planning, interdisciplinary team management/communication, and/or patient care experience.

Other Requirements:

• Ability to multitask with various screen navigation, listening to the participant /caregiver, enter data about the interaction, competency navigating Care Compass and Patient Patterns to locate relevant information.
• Proficient in computer programs such as Microsoft Office, Microsoft Teams, and Microsoft Excel.
• Excellent verbal and written communication skills.
• Accurate attention to detail with strong organizational skills.
• Demonstrated ability to manage multiple projects and be flexible.
• Willingness to occasionally attend in-person activities or meetings as needed at PACE sites.
• Bilingual: Spanish, Russian, Cantonese, Bengali, Mandarin, Albanian, or French Creole.

Physical Requirements

Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to:

• Standing – Duration of up to 6 hours a day.
• Sitting/Stationary positions – Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods.
• Lifting/Push/Pull – Up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc.
• Bending/Squatting – Must be able to safely bend or squat to care for patients, use medical supplies, etc.
• Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas Position requires the individual to be able to travel, and walk between sites/locations and work areas throughout the day.
• Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (ie. typing, use of medical supplies, equipment, etc.)
• Sight/Visual Requirements – Must be able to visually assess patients, read orders type/write documentation, etc. with accuracy.
• Audio Hearing and Motor Skills (language) Requirements – Must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
• Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Salary Range (Min-Max):

Job description

1. To scan accounting documents and upload to PCloud.

2. You will be trained to sort accounting documents into various categories,

such as sales invoices, suppliers invoices, bank statements, petty cash vouchers,

and other cash receipts

3. To perform filing of documents to respective folders.

4. To download documents from PCloud into an accounting software.

5. To visit our clients' premises to do the above tasks.

6. To perform any other jobs that may be assigned to you.

Qualifications.

1. Completed secondary school education

2. Completed NS training.

3. Must be able to speak Mandarin and English

4. Must be able to write Mandarin and English

5. Willing to travel to clients' premises to work

6. Willing to carry a laptop and a portable scanner to clients' premises
