We are looking for a dynamic thinker to support our local planning of patient journeys. The role involves effectively planning patient journeys to and from home / health care setting using our cleric system and against set key performance indicators.

Our patients have varying clinical and mobility needs, some of our patients travel regularly with us into renal and oncology units. Our patients and staff are our priority, ensuring we show care, compassion and empathy.

Phoenix is a well-established Outstanding (Ofsted Nov 2024) special school for students with severe language and communication difficulties including autism. The School offers a unique service to the children of Tower Hamlets. The school has a multi-ethnic population, which reflects the diversity of the borough.
