Management of medicines at discharge from hospital To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g. those with medicine compliance aids or those in care homes). Service Development Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets). Medicines quality improvement Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team. Implementation of local and national guidelines and formulary recommendations Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Assist practices in seeing and maintaining a practice formulary that is hosted on the practices computer system. Auditing practices compliance against NICE technology assessment guidance. Operating arrangements: Responsibility for one or more sites dependent upon patient list size. Based across one or more sites with the opportunity to work remotely from other sites. This is an evolving role and the responsibilities may augment over time in consultation with the post-holder. From time to time the post-holder may be required to work from other sites. Other Tasks Duties may be varied from time to time under the direction of the line manager dependent on current and evolving practice workload and staffing levels. Other administration and professional responsibilities: Participate in the administrative and professional responsibilities of the practice team. Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator. Ensure collection and maintenance of any required statistical information. DUTIES & AREAS OF RESPONSIBILITY Job Responsibilities/Principal Duties to include: Medicines reconciliation from clinic and discharge letters Take a central role in the clinical aspects of shared care protocols Level 1 medication reviews to include: o Identifying patients on repeat medication who have not had a medication review o Check medical notes to assess if any monitoring is required and arrange as appropriate o Update medication as appropriate following defined protocols o Remove any old or unnecessary medication from repeat o Update quantities to provide appropriate supply each month o Review dosage instructions to ensure concise and clear instructions Ensure prescribing is in-line with the Lakeside Drug Formulary whilst taking consideration of CCG prescribing guidance. Support Clinical Pharmacists with general prescription requests and medication queries toallow them to review the more clinically complex requests. Liaise with the pharmacist if there are any concerns regarding patient medication regimens. Promote and support the implementation of electronic repeat dispensing. Conduct searches for audits and high-risk drug monitoring using clinical systems. Participate in practice meetings, and other meetings to improve engagement of the role ofpharmacy technician within the Lakeside Pharmacy model and to promote issues relevant toprescribing. Undertake training and development, to ensure the required knowledge and skills forpractice-based work. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Assist in the delivery of medicines optimisation incentive schemes and audits relating toMHRA alerts, drug monitoring and other patient safety initiatives. Develop relationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team across health and socialcare including primary care, community pharmacy, secondary care, and mental health. Training and personal development: You will participate in any training and development programmes implemented by Lakeside Healthcare. This could include: Participation in individual performance reviews, including maintaining a record of your own personal and professional development. Working with the Senior Pharmacist and the Management team to assess own learning needs and undertaken any training as appropriate. Make effective use of learning opportunities inside and outside the workplace, andprovide relevant feedback where appropriate. Disseminate learning and information gained to other team members in order to sharegood practice and inform others about current and future developments. Undertake mandatory and statutory training as required.

Job summary: The role in the procurement team is to undertake the daily administration on the Trust e-procurement systems. This included training and support for the end users of the system Job responsibilities: To administrate the Trust e-procurement system in an effective and efficient manner. To administrate the Trust e-catalogue system in an effective and efficient manner. To support and administrate end users requirements on the NHS Supply Chain system. To set up and deliver all aspect of training on the Trust procurement systems. Provide ongoing support where necessary to all users of the Trust e-procurement systems. Key Tasks & Responsibilities: To set up, monitor and maintain all new user points on all system platforms. To set up new catalogues In the Trust e-cataloguing system. Export, maintain and support catalogues within the Trust e-procurement system. To link with the Finance department around setting up new suppliers, budget codes and access requests. To monitor, amend and complete purchase orders and requisitions when applicable. To create training guides to support the training of end users. To expedite outstanding order for the end user and procurement team. Communication: To liaise with the Trust e-procurement provider and be first point of contact for the Trust To link with all stakeholders to arrange and undertake training of all end users. To liaise with all users of the system offering support, guidance, share best practice and collaborate with all stakeholders. Respond sensitively and tactfully to queries/complaints and other enquiries relating to the role. Planning and organisational Skills: Arrange training sessions for new users Plan regular auditing of the system to ensure all catalogues are up to date Maintain manual and electronic filing systems for audit requirements. Governance: Be fully conversant with all the procurement policies and contribute to revisions and updates Be fully conversant with all relevant Trust policies and procedures, including, Fire, Health, and Safety, Disciplinary, Grievance and Industrial Relations. Finance and Resources: To monitor and work with the Buyers and Account payable teams to support the matching of invoices to ensure timely payment of invoices. Information Resources: Maintain manual and electronic filing systems for audit requirements. To undertake regular audits of the e-procurement system to ensure all catalogue data is cleansed and updated accordingly, understand the process of raising orders in accordance with Trust Standing Orders (SOs) and the Standing Financial Instructions (SFIs).

Our client has well over 40 years industry experience and has well established contracts and exciting upcoming projects within the Hertfordshire area. Additionally they have contracts across North London and surrounding areas but most notably have an international presence and can offer the opportunity for Engineers to lead projects abroad.

You will be responsible for servicing, maintaining and commissioning cold rooms, walk in chillers, condensing units and more within commercial sites such as factories, food distribution centres and hospitality sites across the South. Additionally you will be required to attend breakdowns and apply emergency repair as part of an on-call rota.
