The essential criteria for you to be considered for this role include: Diploma or degree level professional qualification such as Nurse, OT, Social Worker, Psychological Therapist or Psychotherapist and registered with relevant professional body. 1. To ensure the safe, efficient and correct receipt, storage and distribution of pharmaceuticals. 2. To participate in stock topping up service to wards and departments. 3. The post-holder will work within the Pharmacy Clinical Commissioning Team to ensure all financial and commissioning data for high cost medicines are reported to the Trust Finance and Commissioning Departments to ensure timely billing to external commissioners. This will include ensuring the Trust database is maintained with respect to external reporting requirements including but not limited to patient details, medication (including form, dose, and amount of medication supplied), indication and any other requirements requested by the Trust or the commissioners. In addition, the post holder will assist in the provision of additional Pharmacy databases which are used to inform external commissioners. What youll do: Lead on co-ordination, preparation for and recording of information within multi-disciplinary team meetings. Work with the team to ensure accurate and timely data collection, providing statistical analysis, report generation and any information needed by the team. Prepare and submit performance indicators for regulators and national organisations, including the Quality dashboard and recurrent audits. Maintain adequate records on paper or within electronic records in source documents, central report forms, research contract organisations, data analysers, sponsor companies and auditors. The post holder is an experienced general practice pharmacist, who acts within their professional boundaries, supported and working alongside a team of GreenLight pharmacists in general practice. In this role they will be supported by practice GPs and the GL@GP Director who will develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team and provide clinical pharmacy leadership for the practice, taking responsibility for, and not limited to, Structured Medication Reviews, areas of chronic disease management, medicines optimisation, quality improvement, the quality and outcomes framework and other enhanced services. The post holder will be responsible for leading, training and coordinating the work of less experienced pharmacy professionals to proactively and competently manage repeat prescribing systems, deal with acute prescription requests and medicines reconciliation on transfer of care, if necessary putting in place systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices. The post holder will provide support to practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage GP workload. The role is pivotal to improving quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice.
