Incumbent ensures patients at the Albuquerque Service Unit receive quality, patient-centered care that is timely, appropriate, cost-effective, sensitive and coordinated to affect desired clinical outcomes.

• Provide holistic and comprehensive case management services to all patients that include: intake assessment, benefit assessment, goal setting, long-term care plan development, progress monitoring, advocacy and referrals.
• Serve as the primary contact between referral health care agencies, treatment facilities, patients, patient's families to ensure delivery of effective and appropriate treatment according to established plan of care.
• Develop patient care plans with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s).
• Provide formal and informal health education to patients and family on a one-to-one basis and/or to groups.
• Collect, trend and analyze outcome data to be used in identifying best practices and targeting performance improvement solutions.
• Initiate therapeutic interventions and administers prescribed medications.

Duties include but are not limited to:

• Provides direct care for complex veterans.
• Makes observations of subtle changes in resident's condition, i.e., behavior shifts, mood, skin coloring, appetite, etc. and takes action accordingly within the NA's assigned role.
• Documents care given, observations and responses on appropriate tools or forms.
• Responds appropriately to subtle patient changes by contacting the appropriate nursing personnel.

This position serves as a vital part of the JJP VA Medical Center Police and Security Services often serving as the face of the VA and as the first person a Veteran or visitor encounters when entering a VA facility. The position provides security and assistance to Veterans, patients, employees, contractors, volunteers, affiliates, visitors, and others who frequent VA facilities daily. Major duties and responsibilities include:

• Provides physical security support for Veterans Health Administration, Veterans Benefits Administration, National Cemetery Administration and Veterans Administration Regional Office (VHA/VBA/NCA/VARO).
• Assists in evaluating risk and developing procedures and physical security measures designed to safeguard personnel, property, and operations from unauthorized access, espionage, terrorism, sabotage, damage and theft.
• Ensures security requirements, including appropriate safeguards, cameras, panic and intrusion alarms are implemented.
• Performs law enforcement assignments to prevent or resolve offenses: books the accused; takes the accused before the appropriate District Court or magistrate; confers with the prosecutors; appears before the grand jury upon request; appears at preliminary hearings; and/or testifies at trial, if needed.
• Responds to reports of a crime in progress and/or calls for police assistance.
• Initiates appropriate actions to pursue and apprehend offenders who are leaving the scene of a crime or who attempts to resist arrest. Subdues individuals causing disturbances. Identifies and arrests violators based on eyewitness accounts. Takes charge at crime scenes restricting access to only those persons required to be present.
• Performs bomb search functions as required. Coordinates such search activities with the local bomb squad.
• Assists in managing the Security Program for the Medical Center/Health Care System which ensures that individuals requiring access have been vetted.
• Collects information and prepares reports on the facilities compliance with VA operations, regulations Physical Security and Physical Access Control Systems (PACs).
• Meets with key customers and coordinates with officials to assess customer satisfaction, explain organization policy and procedures, and resolve problems that arise.
• Develops various physical security-related documents, spreadsheets, graphs, reports and presentations using various automated systems and software packages, including the Veterans Health Information and Technology Architecture (VistA) System, Microsoft Word, Excel, PowerPoint to enter, access and retrieve information in the medical center computer system.
• Maintains a Physical Security Tracking report to ensure physical security surveys are followed up promptly.
• Monitors and evaluates existing present and planned security projects to ensure timely and quality service conforms to statutory, contractual and technical requirements.

Functions of Position:

1. Serves as a therapeutic consultant to the Inpatient pharmacy, instructs medical and nursing staff on the chemical, biological pharmacological and therapeutic properties of the full range of drugs and medications.
2. Closely monitored to prevent additional morbidity. This requires specific knowledge and application of biopharmaceutic and pharmacokinetic principles.
3. Consultant to this house staff on medications the incumbent must be able to suggest rational alternatives when requests for non-formulary medications are made.
4. Supply valid and useful drug information.

1. The incumbent will coordinate with and support the unit dose drug distribution system and interact with Nursing Service Personnel and the Medical Staff in resolving all drug related problems.
2. The incumbent must be proficient in screening physicians' orders over-dosage, incorrect strengths, drug incompatibilities, interactions, overlaps, and any other uncommon situations.
3. The incumbent is to provide drug information education and training to pharmacy residents, interns students and in-service training for staff pharmacists and technicians.

1. Have a thorough knowledge of drug stabilities and reconstitution requirements enabling the recognition of incompatibilities, overdoses, and contraindications in prescriptions and doctor's orders. Deviation from established guidelines and procedures may be required after consulting with the prescribing and/or attending physician. The legal responsibility for the clinical work performed and the decisions made, exclusive of direct supervision rests with the pharmacist involved.
2. Responsible for special formulations, compounding procedures and extemporaneous compounding of routine and special medications, this is especially true when particular knowledge is required in a specific area where a precedent or official guideline is lacking.
3. Serves as a reference source and guide to proper I. V. therapy and scientific compounding techniques for physicians, nurses and other members of the health care team.
4. Versed in the latest IV. admixture techniques, IV. equipment and intravenous information references. Integral to this is the understanding of the critical nature of infection control procedures and the importance of maintaining a clean, aseptic medication preparation area.

1. Responsible for actively participating in clinical research projects and coordinating with the Supervisory Pharmacists as these projects impact upon their various sections.
2. As an educational laboratory for resident physicians, medical interns and resident pharmacists provide training, information and technical assistance in the safe and proper use of drugs.

What will I do in this position if hired?

In this position, you will provide medical expertise in support of the FEMA mission to help people before, during and after disasters. Typical assignments include:

• Developing and implementing an occupational health program to ensure workforce medical protection,
• Managing agency vaccination program,
• Establishing remote clinics for the deployed workforce and provide routine and emergency care during disaster,
• Conducting injury and illness surveillance during deployments,
• Managing automated external defibrillator (AED) and trauma kit program.
• Managing CBRN medical countermeasures program for preparedness and response to pandemics and CBRN attacks.

Duties Include:

Clinical Responsibilities (85%):

• Complete psychosocial assessments, and diagnostic interviews to assess the full range of mental health conditions and
  diagnoses. Complete psychosocial assessments to determine the psychosocial functioning, including acuity, high risk or at-risk factors, and needs of the Veterans and/or their families. Must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment to attain the highest level of functioning and independence as possible. Develops a preliminary disposition plan involving the Veteran, and family or significant others.
• Uses social work clinical training, insight, and experience to interpret data to identify treatment options. Performs insightful assessment of serious and complicated cases, makes professional decisions and recommendations for treatment for Veterans, subject to administrator and medical approval.
• Must possess knowledge and ability to independently implement treatment modalities, provide skill-based and Evidenced-Based Psychotherapies in group and individual therapy for Veterans and their families in the BHIP team, and as needed, Specialty Mental Health Programs (e.g. PTSD Clinical Team, Substance Abuse programs, Military Sexual Trauma). At a minimum, must be able to practice trauma-informed mental health care, as the Veteran population has high rates of exposure to trauma.
• Posses a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses, common medications and their effects/ side effects, and medical terminology.

The Medical Support Assistant (Advanced) works collaboratively in an interdisciplinary coordinated care delivery model and performs receptionist duties, customer service and other duties assigned for the proper and timely treatment of patients and maintains appointment schedules for one or more outpatient clinics.

The Medical Support Assistant (Advanced) is responsible for:

• assisting with clinic access contingency plans by adjusting appointment times, location, or dates as well as shifting patients to other healthcare providers as conflict with staffing and/or coverage occurs;
• screening/receiving phone calls in a courteous and timely manner, determining the nature of requests and providing the information desired using privacy rules and established clinic processes;

The primary purpose of this Lead Medical Care Transpiration Technician - Lead Paramedic position is to perform the technical duties as a Paramedic (Basic and Advanced Life Support) and lead other paramedics and Emergency Medical Technicians (EMT).

Measures intraocular eye pressure using techniques including Goldman applanation tonometry
Performs pachymetry to determine corneal thickness & manual keratometry or automated topography to detinning corneal curvature
Performs intra-ocular lens calculations using IOL Master
Performs automated visual field testing using various perimetry machines
Performs external & fundus photography
Performs Optical Coherence Tomography, of anterior segment, retina & optic nerve
Performs urgent ocular patient screening/triages
Educates patients & legal authorized representatives in eye care, including preoperative & postoperative instructions
Educates/trains medical students, residents & fellows
Ensures proper cleaning & disinfection; calibration, maintenance, backup, annual inventory & orders: instruments, supplies & equipment; obtains quotes as needed & reports needed repairs to biomedical engineering
Maintains current BLS course certification
Protects printed & electronic files containing sensitive data in accordance w/ Privacy Act of 1974 provisions & other applicable laws, federal regulations, VA statutes & policies
Protects data from unauthorized release or from loss; alteration or unauthorized deletion
Follows applicable regulations & instructions regarding access to computerized files, release of access codes, etc

Work Schedule: Monday-Friday 8:00am-4:30pm
Paid Time Off: 37-50 days of annual paid time offer per year: 13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year

The Social Worker duties include, but are not limited to:

• Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
• Assesses and identifies high-risk veterans and follow protocol for suicide prevention.
• Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
• Establishes and maintains effective therapeutic relationships with veterans and their families.
• Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to the treatment plan.
