This role is an early intervention and preventative post based in the Family Centred Wellbeing Service (FCWS), within our H&SCP. The successful candidate will provide assessment and evidence based intervention for children/young people aged 5 -18 years to enable them to achieve their full potential across all aspects of daily life at home and in the community. The role will also encompass consultation, advice and support for children/young people and their families and the wider multi-agency/disciplinary team. The FCWS has been identified, along with other services, as a support service for children/young people and their families where there may be a neurodevelopmental presentation (with or without a diagnosis) impacting on physical, social, emotional, behavioural and/or mental health and well-being. Therefore, a good working knowledge of current national policy and relevant experience of working with children/young people would be advantageous, however, this is not essential as training will be made available. It takes a Village to care for our patients and as an IKC Medical Office Assistant/ Care Coordinator (CC), you will be at the heart of all the action.  Our CCs work together as a team to facilitate the planning and organizing of all critical activities for our patients.  The patients we care for are some of the most complex patients and need an excellent care team to help manage their chronic conditions.  The Care Coordinator works closely with our Nurse Practitioners, RN Case Managers to make sure our patients receive the highest quality care.
 

Position Details:

• Assist the clinical team with moving the patient through the continuum of care by communicating with and completing tasks as directed.
• Assist the care management staff with implementation and documentation of the discharge care plan that includes initiation of referral forms, gathering clinical information, initiating the process of completing a chart and appointment scheduling.
• Communicate directly with patients on a timely basis for all scheduling requirements.
• Maintain accurate and complete documentation of records. Forms are fully-completed, legible, have no unapproved abbreviations and have the proper signatures.
• Ensure that documents get to appropriate parties in a timely manner, without compromising confidentiality or patient privacy procedures.
• Demonstrate exceptional two-way communication using clear and well-organized oral and written techniques.

Qualifications of the Medical Office Assistant:

• High school diploma or GED required
• Experience in a medical office or hospital setting a bonus
• At least two (2) years of experience in a professional office environment

Major Duties

Oncology Formulary Management and Utilization:

• Manage the oncology formulary by evaluating and onboarding new agents.

• Determine the appropriate site of care for patients receiving oncology treatment, considering factors such as inpatient, outpatient, or regional campuses.

• Oversee the review process for off-label, non-formulary, or other non-standard use of antineoplastic agents.

• Promote appropriate, standardized and guideline-based antineoplastic utilization across all El Camino Health (ECH) sites where appropriate.

• Review provider requests for exception orders and evaluate for literature support to guide the prior authorization process when applicable.

• Collaborate with relevant stakeholders, including physicians and other healthcare professionals to ensure the safe and effective use of these agents.

Collaboration with Pharmacy Informatics Teams:

• Work closely with pharmacy informatics teams to contribute to the development and revision of Electronic Health Records (EHR) resources.

• Ensure that EHR resources support the safe and effective use of oncology medications and adhere to current guidelines and best practices.

Education:

• Coordinate, facilitate, and provide education, training, competency assessment and performance improvement to pharmacists, pharmacy residents and other healthcare professionals for the appropriate use of antineoplastics as well as on best practices and the latest developments in oncology pharmacy.

• Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.

Quality and Performance Improvement:

• Contribute to the institution’s antineoplastic stewardship program through compiling data and metrics on antineoplastic utilization in the outpatient and inpatient settings, reporting data and findings to relevant stakeholders.

• Monitor antineoplastic utilization practices to identify areas for improvement and implement necessary changes.

• Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.

• Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.

• Stay updated on the latest advancements in oncology pharmacy and ensure that the pharmacy services align with current standards of care.

• Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all ECH sites and promote and enhance patient outcomes through safe, effective, cost-effective, and appropriate medication therapy use for patients receiving antineoplastic therapy.

Qualifications – All

• Doctorate degree pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.

• A one (1) year accredited PGY1 (Post Graduate Year) pharmacy residency.

• A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with minimum of 2 years of oncology experience that includes pharmacy regulatory compliance, compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.

License/Certification/Registration Requirements/Credentialing

• Valid California state pharmacist license with no disciplinary action(s) against the licensee that prohibits the licensee from practicing pharmacy.

• Valid CA driver’s license

• Valid NPI (National Provider Identifier) number issued by the Center for Medicare and Medicaid Services (CMS)

Ages of Patients Served

This position is responsible for the assessment, treatment, and care of adolescent, adult, and geriatric patients.

$75.09 - $112.64 USD Hourly
