Working as an integral part of the Faster diagnosis team (FDT) across Somerset. The post will ensure that patients referred on a suspected cancer pathway are informed that they have cancer or that cancer is excluded within a maximum of 28 days from referral. The post holder will work collaboratively with primary and secondary care, clinical and operational teams, to ensure patients receive a high quality, safe, effective, and timely diagnostic pathway. The role will be to manage the Urology/Gynaecology Faster Diagnosis Team across Somerset, ensuring that patients referred on a s suspected cancer pathway are triaged to the appropriate investigation/outpatients and support patients being told they have cancer, or that cancer is excluded, within a maximum of 28 days from referral. The post holder will also undertake telephone clinic for patients who are aged >85 and/or with a Rockwood >5 to be able to provide an objective and balanced assessment of an individuals circumstances, conditions and wishes to be investigated. Working in conjunction with the lead clinicians for specific speciality, you will further develop and maintain the delivery of best practice in the care and management of cancers from referral or diagnosis. The post holder will act as a referral and liaison point for the MDT and as an information resource for all those (in both primary and secondary care) who are involved in the care and management of malignancies, thus ensuring the provision of a seamless service for this group of Somerset patients. The lead faster diagnosis nurse requires practice that is informed by higher levels of judgement, decision-making and discretion. The lead FDN should play a significant role in setting standards/ guidelines, undertaking audit and research, educating others, and disseminating good practice. The post holder will be expected to provide positive and inspiring leadership to the team to maintain and develop a culture and environment that allows the team to deliver the best possible standards of clinical care, experience and employee engagement. We are continually looking to review and improve pathways and the offer to patients. The post holder will be working for Somerset Foundation Trust, so will be required to work at both Musgrove Park Hospital and Yeovil District Hospital, and it will require onsite working (weekly) as well as remote working.

We are looking for a practice nurse to provide general nursing treatment room duties to all patients registered to the practice with scope of further training in chronic disease management and personalised care. The post Holder is an experienced nurse, who acting within their professional boundaries, will provide care for the presenting patient. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. The role will be to ensure patients triage is undertaken promptly to allow for investigations to be carried within a timely manner. This may involve telephone consultations and desktop triage. Facilitating straight to test/appointments to assist in the delivery of results to the patient within the 28 days. For patients with results highlighting a cancer or potential cancer, you will work closely with cancer services and the speciality specific teams to ensure a swift transition to the responsible team

To provide effective clinical nursing skills to the ED Department during the shift around a 24 hours period. Part of the team which empowers staff to deliver patient- centred, evidence based nursing care within a defined budget. Support the systems for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy. Delivers patient care including health promotion on a daily basis to emergency patients who present with undifferentiated conditions including life-threatening emergencies to adults and children and will present unscheduled and unannounced at any time throughout the 24 hour period. Ensure line manager/hospital management aware of staffing short falls, bed shortages/department overload, responding to requests for information to enable them to ensure safe resolution.
