We are looking for an enthusiastic andforward-thinkingBand 5 nurse to complete ourHealth Advisor/ Clinical NurseSpecialist team.This isa developmentalpost,post holder will be supported to complete training in all aspects of sexual healthwithin atwo-yeartimescale.

We are a busy level 3 sexual health service based in Slough. We offer sexual health services in Windsor, Maidenhead, Ascot, and Bracknell via our satellite service. We work on a Monday to Fridaybasis, working 1 Saturday every 4 to 6 weeks. 


Due to the nature of this role, we are looking for those who are open-minded and non-judgemental, with the ability to work in a highly confidential environment.Previousfamily planning or sexual health experience within the GUM setting is essential for this role. 


You will have the opportunity to be part of a multidisciplinary team, where we offer incredible training opportunities, such as non-medical prescribers training. 


 Working within Sexual Health, no day is the same and you will be met with a variation of cases from testing and treatment for sexually transmitted infections, including HIV, and contraception.  Giving you the exposure to develop and enhance your current skillset.

The post holder willbe responsible forproviding a high standard of sexual reproductive healthcare to patients accessing the sexual health service. The post holder will be expected to work as part of a multi-disciplinary team offering holistic, non-judgemental sexual health caredemonstratingprofessional and ethical responsibilities and to build andmaintaingood working functional relationships with all members of the multidisciplinary team. The post holder will be responsible for the management of the pathology results and to ensure that patients are provided with information, advice, treatment, and support when they are diagnosed with a sexually transmitted infection The post holder will be expected to supervise and mentor junior staff and nursing students, and to take an active rolein the leadership and management of the nursing team. This post involves working at the Garden Clinic in Slough, but you maybe requiredto travel to work at locality clinics in Bracknell, Windsor, and Maidenhead. Facilitate and co-ordinate the care pathway of patients on disease modifying

treatment (DMT) by:



· Ensure relevant investigations (e.g. MRI, CSF, blood results) are available to Consultant Neurologists and nurses in the DMT clinic to enable comprehensive assessment and initiation of most appropriate DMT.

· Complete high-cost drug notification forms as required.

· Establish and ensure the continuation of effective communication between the MDT team, external homecare delivery companies, injection support nurses, regional neurologists, regional MS nurses, GPs and the patient to enable seamless provision of DMTs.

· Preparing the list of prescriptions sent in by homecare companies and obtaining recent blood results from GP surgeries if they are not on EPIC and uploading.

· Monitoring patients’ attendance, following up as appropriate, particularly treatment-naïve patients and those taking DMT and ensuring 6 monthly reviews with clinicians.

· Telephoning patients with identified cognitive difficulties to remind them of OPA’s thus reducing DNA’s.

· Printing and sending patient DMT letters

On Eden Ward we care principally for haematology patients and patients undergoing investigations and treatment of disorders affecting the blood or bone marrow. We also care for patients with different types of cancer. We provide high standard of care to patients receiving intensive chemotherapy treatments on a daily basis. This is an exciting time as we are planning on starting a new autologous stem cell transplant service at Wexham Park Hospital as a spoke for Oxford University Hospital.

Driven to provide nursing led care you will be working with a dedicated patient list to provide holistic care to our patients and as part of the multi-disciplinary team ensure their safe and timely discharge. We are excited to be using an E-prescribing system and provide full training on this. You will also work closely with the Eden Day Unit.

We have a dedicated practice development team to drive your professional development so wherever you are in your career journey we can help you grow. Once experience and knowledge are built, you will be offered chemotherapy training to ensure safe administration.

Chemotherapy training will be provided, however, there will be a waiting list so will not be immediate

Provision of nursing care for patients with a variety of acute conditions

An exciting opportunity has arisen for a highly motivated senior nurse who is passionate about Haematological cancer to lead and develop the cross site Haematology Cancer CNS Team. This is a permanent Band 8a post at 37.5 hours per week. This is a Macmillan badged post with access to support and educational funding.

They will play an active part in clinical practice, education, research/audit and the implementation of national guidelines and transformative services.

The successful applicant will work as part of a well-established dynamic multi-disciplinary Haematology Cancer team who provide specialist advice, holistic care and support to patients, relatives and members of the health care teams. They will be required to lead, develop and evaluate the Haematology Cancer service to improve and maintain standards of patient care and promote a seamless service within a multi-disciplinary framework.

The post holder will lead and manage the Haematology Cancer CNS Nursing Teams cross-site on a daily basis, ensuring that the teams provide an excellent service to the Trust through evidence based practice, education, policy and guideline development.

Provide leadership and support for the Haematology Cancer CNS Teams cross-site

Work autonomously and demonstrate advanced practice by managing a caseload of patients and demonstrate advanced practice in the holistic assessment

Lead the development of Personalised Care for haematology cancer patients which will include the further development of the Supported Self Management programme and play an active role in the delivery and design of ‘Moving On’ and Health & Wellbeing Events

Promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist area

Facilitate effective communication and co-ordinate referral of patients and carers to appropriate support services and community Trusts to promote a seamless service and thus reduce/avoid admissions and length of stay to the acute trust
