Hi, I’m Hannah and I have been working as a Band 6 Specialist Respiratory Nurse in the Community for 5 years now and honestly, I can’t imagine working anywhere else. We are looking for a Band 6 Respiratory Specialist nurse to join our team.
Our team works with patients in the community to support them at home with their chronic respiratory conditions, as well as in their pulmonary rehabilitation rehab journey. For me one of the most rewarding parts of the job is being able to support patients to improve their quality of life, because it allows you to develop closer therapeutic relationships with your patients. In our team we have Specialist Nurses, Physiotherapists and Respiratory support workers, and we work closely with the wider Multidisciplinary Team.
I moved to this team from a secondary care setting and was anxious about the move, but it has absolutely been the best decision I’ve made. I can’t see myself leaving this job unless I win the Euromillions. I love supporting patients to take control of their own care and gain their independence.
We are looking for a qualified and registered Nurse, but our goal is to find someone with passion and interest in respiratory conditions and supporting people in their own homes. Therefore, we are happy to consider candidates without community and/or respiratory training.
If you are successful, you will be working with patients with chronic lung conditions to deliver Pulmonary rehabilitation and specialist respiratory care. You will be completing assessments for patients, developing care plans, and implementing and evaluating interventions. You will provide oxygen assessment and management. You will also support in providing quality end of life care.
The successful candidate will be supporting the team by managing, promoting, and delivering our Pulmonary Rehabilitation Programme with classes running throughout the county. The classes are a fantastic way to get to know and support your patients and we love seeing them discover their independence in managing their illness, with vast improvement to their quality of life and symptom burden.
We work collaboratively with colleagues in Primary and Secondary care services and other providers who deliver Pulmonary Rehabilitation in Cambridgeshire and Peterborough. • To manage a caseload of patients with complex respiratory needs, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions
• Working within the largest provider of Pulmonary Rehabilitation in the county, you will assist in managing, promoting, and delivering the Programme. Ensure that this is delivered, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.
• To support the application of CPFT’s Pulmonary Rehabilitation Accreditation.
• Management of advanced respiratory disease and promotion of self-management
• Oxygen assessment and management with support of senior members of staff.
• Quality end of life care
• To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
• To participate in multidisciplinary team (MDT) working and provide specialist advice, support, and education to clinicians.
