What You’ll Be Doing:

The Transitional Support team are looking for an enthusiastic and motivated individual to join their team. The specialist nurse is responsible, with minimal supervision, for the assessment of care needs and the development, implementation and evaluation of care provided for patients with haemophilia and inherited bleeding disorders. The Churchill pharmacy and renal team are looking for an experienced Pharmacy Assistant looking looking to expand their skillset with a move from Community into Hospital Pharmacy, or are looking to progress further in your career within a Hospital setting. Ideally you will have obtained your Specialist BTEC Level 2 Principles and Practices for Pharmacy Support Staff Pharmacy Service Skills NVQ Level 2 (or equivalent GPhC recognised qualification), although this is not essential and support through the qualification process can be provided (successful applicants who do not hold the required qualification will be paid at band 2, rising to band 3 on successful completion of the qualification). Many of our staff progress from this level to become Pharmacy Technicians or other roles within the wider Pharmacy team. The PACT ICT is responsible for providing direct and/or indirect patient care to an assigned group of patients. She/he will have the knowledge, skill and experience to carry out assignments involving point of care testing, providing treatments and vaccinations, performing diagnostic studies, and patient/family education. The ICT demonstrates active collaboration and communication with Patient Aligned Care Teamlet (Patient, Primary Care Provider, RN Care Manager and Administrative Associate) and Larger Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient). This is the full performance level for the ICT position. This is a bargaining unit position.

Major duties include:

• The incumbent assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations.
• Performs observation-based screening and collecting patient information in collaboration with licensed personnel.
• Completes approved templates and tools, such as the outpatient intake form, normal result notes, and follow up treatment protocols established by the agency.
• Assists in behavioral/psychiatric situations in an outpatient setting. This can include assisting licensed personnel with crisis intervention, management of aggressive or suicidal psychiatric patients, by providing one-to-one observations and applying prevention and management of disruptive behavior techniques (PMDB).
• Performs duties focused on communication techniques required to complete the questioners and templates approved for use in any outpatient clinical setting, including active listening, restating, and mirroring.
• Maintains a safe and therapeutic environment for the patient while promoting a sense of worth and dignity under the supervision of the licensed personnel.
• Assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations.
• Assists licensed staff with preparing patients for medical evaluation, setting up equipment, passing instruments and providing treatment during in person or virtual visits.
• Operates specialized medical equipment to obtain diagnostic and therapeutic outcomes.
• Collects a wide range of specimens and preparing them for transfer to processing.
• Observes patient status and keeps licensed clinical staff informed of any change in condition.

  • Preparation of lab specimens and transfer to the lab for processing: Examples: Labeling, placing in the proper bin or tube system.
  • Assisting with patient exams and screening, utilizing technology such as telehealth peripherals. Examples: primary care physical exams (heart, eye, ear, nose, throat (EENT); gait exams, and skin exams.
  • Patient education within scope of care or while providing an intervention. Examples: post procedural care, splint care and wound care.
  • Follow-up care: Examples: Suture removal.

The Lead Nurse for the Thames Valley and Wessex Paediatric Critical Care ODN will provide nursing leadership across the Network, acting as a source of expertise, advice and knowledge for providers, commissioning and ICBs. You will work alongside the national PCC ODN lead nurses to deliver on nationally agreed programmes of work and to support regional surge planning for PCC capacity.

You would work with the Network Manager, Education team and Lead Clinicians as part of a multi-disciplinary leadership team to lead the strategic and operational development of coherent and effective network arrangements, and delivery of a programme of work that addresses national and local priorities, leading on user engagement and risk management. some of the key programmes of work for the upcoming 18 months are: implementation of the PCC services specifications for Level 2, Level 3 and Transport services, working in collaboration with LTV providers to develop services alongside the NCEPOD recommendations and implementation of national Paediatric Early Warning scores.

You will also, in partnership with the ODN Education team, PIER and SORT play a key role in delivering education and training for Paediatric Critical Care across the ODN.

• Provide proactive support to the patient support groups that are established within the network, ensuring appropriate links with national charities and third sector organisations.

Your core role in Clinical Trials Pharmacy would be to:

Obtain and maintain Good Clinical Practice (GCP) training, apply principles of GCP in the management of clinical trials and work within the Medicines for Human Use (Clinical Trial) Regulations 2004 and EU directive relating to Clinical Trials, communicate with Investigators, Research Nurses, Trust R&D, Sponsors, Clinical Research Associates and Pharmacy staff during the set-up and running of a clinical trial, ensure clinical trial protocols and SOPs are followed during dispensing of clinical trial investigational medicinal products and other drugs included in the protocol, order and receive in clinical trial drugs, process receipt paperwork and acknowledge receipt on IVRS (Electronic Medication Supply) systems, check stock levels and expiry dates of clinical trial drugs and keep balance logs up to date, assist with the packing down and re-labelling of clinical trial drugs if required, assist with the preparation for monitoring meetings, audits and inspections, maintain Clinical Trial files ensuring items can easily be located and are presentable, process returned clinical trial drugs and ensure the paperwork is kept in order, participate in the statutory temperature monitoring of clinical trial stocks, assist with the destruction of clinical trial medication, assist with the closedown and archiving of clinical trial files, represent the Clinical Trials Team to update the dispensary staff at meetings, liaise with Research Nurses to update them on trial progress, stock levels etc, assist in the collection of data as requested by the Senior Clinical Trial Technician or Clinical Trial Pharmacists, assist in maintaining financial records, including the generation of invoices, checking payments etc, assist with general administration such as organising set up meetings etc and be a point of contact within the Clinical Trials Pharmacy Team, maintain clinical trial filing systems and drug accountability documentation for clinical trials, use the computer system and other information technology in all areas of Pharmacy.

You will required to assist in the running of specialised clinics such a our 2 week wait hysteroscopy, vulval and pelvic mass clinics, our colposcopy clinics supporting clinicians carry out treatments following abnormal smears and our general gynaecology clinics that include urogynaecology and endometriosis clinics. This role covers a wide variety of services within women’s health. You will be part of the nursing team to support the department sister in the running of the unit. You will assist in the maintenance of the patient environment in terms of cleanliness, tidiness, auditing, and the restocking and ordering of supplies. You will be part of the team also supporting our Pre op assessment nurses to carry out assessments on our patients that fit certain criteria following out patient hysteroscopy being booked for general anaesthetic.
