We currently have a post available in our busy and constantly evolving Ophthalmology Day care unit- department and we are looking for enthusiastic, motivated and dedicated Registered Nurse. Band 5 to join our team of nurses and staff.

The DCU on Alex Cross ward to deliver special care for ophthalmic surgery including elective and emergency services. Patient care/ day care unit facilities to these patients admitted at Western Eye Hospital. The Western provides a comprehensive range of general and specialist ophthalmic facilities and is equipped with some of the most advanced diagnostic and technical equipment currently available.

The Western Eye Hospital forms part of Imperial College Healthcare NHS Trust in the heart of Paddington, an area which is set to become one of the capital’s most attractive and vibrant centres.

In this role you will provide the best possible nursing care to patients with a wide range of ophthalmic conditions. A new and exciting opportunity has arisen for a highly motivated and enthusiastic pharmacist with excellent clinical and interpersonal skills to lead and deliver a high-quality service to the Integrated Care Operations (ICO) service at University College London Hospital (UCLH).

The ICO oversees a range of services focused around safe and efficient transfers of care. These include:

UCLH@Home, which provides a home nursing service to enable safe transfer home for patients under the management of a UCLH Consultant.

The Integrated Discharge Service (IDS), which in close collaboration with North Central London (NCL) Integrated Care Services, provides an Admission Avoidance service to the Emergency Department through pathways such as Camden and Islington Virtual Wards and other more bespoke services including the Camden Adult Pathway partnership (CAPP), a Homeless Intermediate Care service and a Rough Sleeper Nurse working with the Camden Street teams.

The post holder will work as part of the Medicine and Emergency Services pharmacy team to deliver pharmacy services and safe transfer of care between the ICO and other NCL Integrated Discharge Services.



Duties will include co-ordination of and participation in the day-to-day clinical pharmacy service to the ICO services team and attendance at relevant huddles and handover meetings. You will also work with the ICO services team on projects to develop, implement, maintain and improve new services.

You will use your independent prescribing skills to optimise medication regimens in preparation for discharge and act as a point of contact for staff and patients with queries relating to their medicines.

An exciting opportunity has presented for a pharmacist to join our cancer pharmacy team. We are looking to recruit to the post of Rotational Pharmacist in Cancer who will play a pivotal role in managing the pharmacy service provided to cancer patients.

UCLH, in partnership with University College London, has been designated one of the country’s five comprehensive biomedical research centres and there is an on-site early phase cancer clinical trials unit.

You will lead the day-to-day clinical pharmacy service to the UCLH SDEC, attending clinical rounds and liaising closely with multidisciplinary team (MDT). You will also attend relevant planning huddles and handover meetings.

You will use your independent prescribing skills to optimise medication regimens and prepare plans for discharge. You will support the MDT as a point of contact for staff and patients requiring information about their medicines in all care settings.

Through service evaluations and quality improvement projects, you will work to develop and enhance our existing systems and processes, particularly where safety, efficiency, and productivity can be improved through our Electronic Health Records System, Epic.

You will develop, implement and maintain clinical pathways that cross care boundaries, including those that involve complex and high cost drugs, liaising with colleagues in other organisations across North Central London (NCL), and in primary care. You will also support patients on admission avoidance or early discharge pathways that utilise virtual wards.

The Carelink Service is a team of Community Health Support Workers who provide care within the boundaries of Camden for adults over 18 although most of the clients are older adults. The service predominantly provides care between the hours of 7am and 10 pm seven days per week. However, the service also provides a night sit in service from 10pm to 7am. Carelink is part of Camden Integrated Adult Service (CIAS) which is a physical health integrated service run by CNWL, supporting GPs and hospitals. Working together, the different teams in the service provide health care for adult patients in Camden, including rapid response for admission avoidance, discharge to assess and palliative care. We also provide a Phlebotomy service to clients who are house bound in the Camden community.

The post holder will be working in the community to provide enabling care and promoting independence and/or providing end of life care with the client in their home environment and to undertake visits to client’s homes independently or with other team members as and when required.

Please note this is not a mental health support worker role.
