CLINICAL After a period of training, complete a full clinical evaluation and assessment, including history taking and comprehensive physical examination of all patients undergoing elective cardiothoracic and cardiology procedures, identify appropriate investigations, including X-ray and Echocardiogram. Interpret and act upon complex clinical findings and investigation results, including discussing with or notifying consultants, GP or other healthcare providers. The core responsibilities of the role involve initiating individualised plans of care, addressing any complex health and well-being needs. Refer to, liaise with, and maintain effective communication with patients, carers, other members of the multi-disciplinary team and primary / secondary care services. Assess patient/ family understanding of information provided using appropriate techniques. Identify complex patients who are at high risk of re-admission and escalate accordingly. Plan for a safe and timely discharge with patients, family and carers. The CNP provides medicines management advice to patients and their families/carers at all points of the pre op pathway, e.g. before the patient is seen in clinic or following their pre assessment. The practitioner must also demonstrate an understanding of competing and sometimes complex medication regimes including anticoagulation advice, and how to adapt these for individual patients in collaboration with the assigned consultant. This requires a basic understanding of commonly used drugs in cardiothoracic/cardiology services. The successful candidates will work towards completion of non-medical prescriber training or reactivation of current qualifications in line with Trust policy. Following appropriate training, the nurse practitioners also consent patients for identified procedures on behalf of the supervising consultant. Provide a telephone help-line service for advice and support for patients and their families waiting for and following procedures. Produce, update and contribute to patient information literature, policies and protocols which are Trust wide in their scope. The team collaborate with and contribute to Trust wide projects that impact the pre op pathway. Contribute to the development and management of nurse led services and role development in line with strategic targets and national and local guidelines.CLINICAL QUALITY Utilise an evidence based approach to the management of patient health and illness Ensure practice complies with trust policies, procedures and clinical guidelines Engage in clinical supervision, self-reflection, audit and quality initiatives that contribute to improvements in care and practice Analyse audit information to assess activity and quality and share across relevant forums. Collaboratively work across health care sectors to improve the patient experience and maximise the efficiency of service delivery. Take a proactive approach to planning the CNP workload to ensure smooth running of service. Co-ordinate and manage the daily running of the clinics, negotiate with clinical and non-clinical personnel. Provide specialist, expert clinical knowledge to colleagues, patients and carers/relatives. Participate in professional and policy making activities that contributes to the development of the CNP role and the health and wellbeing of patients. We are looking for an enthusiastic and motivated person to fulfil the post of Early Diagnosis Cancer Support Worker, working within our Non Specific Rapid Diagnostic team.

You will work extremely closely with our Advanced Clinical Practitioner Nurse Specialists and assist, support and co-ordinate a smooth patient journey and pathway.

A large part of the role is providing administrative and co-ordination support to patients, families, consultants, cancer nurse specialists and other staff involved in the early part of the cancer pathway.

The successful applicant will ensure the efficient channelling and close co-ordination of the outpatient pathway ensuring robust communication between medical staff, GPs, patients, relatives, members of the public and other disciplines within the Trust.

The post holder will demonstrate an understanding of cancer services, have excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

The post holder will have excellent communication skills and be willing to undertake ongoing training and development. In addition, the post holder will be able to signpost patients and carers to the relevant sources of support and information appropriate to their needs.

What you’ll be doing:

Preparing fire, health and safety risk assessments in accordance with legislative requirements.
Provide robust fire, health and safety advice to clients with high level advocacy whilst delivering of the ‘RPS Way’.
Develop emergency plans and evacuation plans (including specific tenant plans).
Deliver on client specific projects and competent person roles.
Develop retrospective fire strategy reports for residential and student accommodation buildings.

Participate in team huddles, provide direction, and mentorship to other consultants.
Support Technical Director in department projects.
Complete technical reviews of consultant fire, health and safety risk assessments.

Identify and discuss solutions to health, safety and fire related problems.
Respond to technical concerns and problems, ensure smooth implementation and ongoing client support.
Resolve any problems in a timely manner.
Maintain own professional CPD on a regular basis, open to professional development through external courses and qualifications to serve company in business project direction.
Review international regulations and compliance regulations, complete European health, safety, and fire safety risk assessments as required.

Excellent literacy skills and technical knowledge of fire, health, and safety legislation for residential commercial, manufacturing and retail sectors.

Familiar with BAFE SP205 Validation process.

Demonstrable CPD record.
Member of IFSM or IFE.
Recognised Fire Risk Assessment qualification.
Diploma level qualification in health and safety.
CMIOSH or equivalent (or working towards).
Registered on a recognised Fire Consultants’ Register.
Thorough understanding of British Standards relating to fire in the residential sector.
Proficient in practical application of health and safety legislation.
Building regulations Approved Document B, British Standards, relevant fire, health and safety legislation, ACOPS, Approved Documents M and K.
Awareness of CDM 2015 Regulations.
Desirable:

Type 4 Fire Risk Assessment and DSEAR experience desirable.
