The post holder is responsible for the delivery of basic practice nursing service care to the practice population. Supported by Nurse Practitioners within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership and mentorship as required. Core Clinical Function FREED Champion To manage all referrals accessing the service via the FREED pathway, including initial triage and subsequent allocation. - Lead a weekly FREED Huddle within the Community Team meeting bringing together all clinicians involved in the FREED pathway Assess patients referred to the service and co-produce appropriate treatment plans. Access appropriate evidence-based treatment such as MANTRA, CBT-E, SSCM etc. To continuously review the implementation of the FREED service model and care package, advising the Service Lead on its progress and identifying any areas for improvement or opportunities for further service development. To be responsible for ensuring that required core outcome data are collated and reported back to the FREED network. To proactively engage with the FREED project and join the FREED supervision groups. Hold responsibility for co-ordination and management of the FREED project in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high-quality care packages. Core Clinical Function Keyworker Manage and oversee the care of service users with highly complex needs. Manage a caseload utilising person-centred assessment, planning and implementation of care plans. Evaluate care plans and outcomes including active participation in clinical audits and outcome measures. Work closely with service users and their families/loved ones to ensure their wishes and needs are heard and understood. - Work closely with the service users and our support worker to ensure a holistic approach to enhance quality of life and confidence in activities of daily living and integration in the community to prevent isolation. Core Clinical Function Eating Disorder Therapist Assess clients referred to the Community Eating Disorder Service and provide appropriate treatments. Provide evidence-based therapies for people with an eating disorder i.e. CBT, CBT-E, CBT-T, MANTRA, SSCM Attend the weekly Community Team meetings to discuss cases, offer clinical opinions and support team. Deputise for Service Lead in their absence and represent the team as required. Core clinical Functions all roles Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation/records on all service users in line with Navigo policy. Conduct, supervise and oversee risk assessments of all service users. Provide professional clinical leadership to multi-disciplinary meetings i.e. referral/review/business, professional or safeguarding meetings. Report and document incidents as required and act upon findings as appropriate. Provide practical support, guidance and education for service users, carers and other professionals as required. Be competent in use of relevant screening and assessment tools. Encourage completion of outcome measures to allow for service evaluations. Lead and participate in formulation, delivery and review of individualised evidence-based care/care packages according to NICE guidance including non-medication strategies. Manage and co-ordinate multi-disciplinary meetings ie referral/review/business/handover. Provide clinical leadership and professional expertise to the team. Deliver training where appropriate for other professionals. Management Responsibility You will be expected to support the work of the Service Lead in managing the performance of the service. This will include resource management, operational planning, clinical governance, and workforce planning. You will be expected to support and plan new service initiatives taking a lead management role in these initiatives as requested by the Service Lead or operational service manager. You will be expected to be involved with the review and implement of any relevant policies. Ensure that the service through the implementation of performance management, all key performance indicators and targets identified by Navigo are met and adhere to local and government initiatives.

Job Purpose This is a training role within the NHS England service. The post-holder will work within the NHS Talking Therapies service providing high intensity interventions whilst undertaking a programme of training for this role. This intensive training post will equip the post holder to provide a cognitive behavioural therapy (CBT) service to clients with a range of complex problems for which CBT is demonstrated to be clinically effective. The post holder will work in the service for three days of the week using the newly developed skills whilst attending the training programme for the other two days. The post holder will work flexibility within the operating hours of the service, and progress to Band 7 upon successful completion of the course and application is made for accreditation with the BABCP. Main Duties and Responsibilities 1. CLINICAL 1.1. Accept referrals via agreed protocols within the service 1.2. Assess clients suitability for Cognitive Behavioural Therapy and follow the relevant pathways, if not appropriate 1.3. Make decisions on suitability of new referrals, adhering to the services referral protocols, and refer clients on to the relevant service or back to the referral agent as necessary. 1.4. Formulate, implement and evaluate therapy programmes for clients. 1.5. Educate and involve family members and others in treatment, conveying CBT formulations with sensitivity in easily understood language. 1.6. Adhere to an agreed activity contract relating to the number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient. 1.7. Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate. 1.8. Complete all requirements relating to data collection within the service. 1.9. Keep coherent records of all clinical activity in line with service protocols 1.10. Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach. 1.11. Assess and integrate issues surrounding work and employment into the overall therapy process 2. TRAINING AND SUPERVISION 2.1. Attend and fulfil all the requirements of the training element of the post. 2.2. Apply learning form the training programme in practice 2.3. Receive supervision from educational providers in relation to course work to meet the required standards. 3. PROFESSIONAL 3.1. Ensure the maintenance of standards of practice according to the employer and any regulating, professional and accrediting bodies (eg BPS, UKCP, BABCP), and keep up to date on new recommendations/guidelines set by the department of health (e.g NHS plan, National Service Framework, National Institute for Clinical Excellence). 3.2. Ensure that client confidentiality is protected at all times. 3.3. Be aware of, and keep up to date with advances in the spheres of CBT 3.4. Ensure clear professional objectives are identified, discussed and reviewed with senior therapists on a regular basis as part of continuing professional development. 3.5. Attend clinical/managerial supervision on a regular basis as agreed with Manager. 3.6. Participate in individual performance review and respond to agreed objectives. 3.7. Keep all records in relation to Continued Professional Development up-to-date and ensure Personal Development Plans contains specialist knowledge of the latest theoretical developments. 3.8. Attend relevant conferences / workshops in line with identified professional objectives. 3.9. Be involved in the positive promotion of NHS Talking Therapies to other services and the local community, taking ownership of the services success by monitoring their own performance. 4. GENERAL 4.1. To contribute to the development of best practice within the service. 4.2. To maintain up-to-date knowledge of legislation, national and local policies and procedures in relation to Mental Health and Primary Care Services. 4.3. ADVISORY / LIAISON Provide an advisory service on matters related to the practice and delivery of Cognitive Behavioural Therapy or other therapy to individuals/groups/committees across the Mental Health Trust, Primary Care Trust and other voluntary agencies. Promote and maintain links with Primary Care and Secondary Care Staff to help co-ordinate the provision of an effective Psychological Therapies Service. We have an exciting development opportunity for a Band 8a Advanced Clinical Pharmacist with a speciality in Women and Children at Northern Lincolnshire and Goole NHS Foundation Trust (part of the Humber Health Partnership). This may be offered as a Band 7-8a progression role depending on experience.

You will undertake a structured process, receiving additional support from the established team at Hull Royal Infirmary, to achieve Advanced Clinical Pharmacist status and, upon successful completion of your objectives, demonstrate high quality acute clinical pharmacy skills, together with leading a team. A ward-based role, working flexibly to deliver activities across 3 major acute sites within the Group. Most of the activities will be at our Grimsby and Scunthorpe hospital sites, and occasional requirements to travel to our Teaching Hospital at Hull Royal Infirmary (depending on service demands) whilst participating in rotas and/or shift working as necessary, including weekend and bank holidays.

Base site will be agreed upon job offer, and travel expenses (or alternative travel arrangements) will be provided for travel between the Group sites. Please note that there are no public transport links between the Group sites; the ability to drive is essential for this role.

The postholder would be responsible for providing an advanced clinical pharmacist service within the Women and Children Directorate. This role requires excellent organisational and interpersonal skills alongside sound clinical knowledge and relevant experience. We are a successful, innovative and forward-thinking department with patient care at the centre of decision making. We actively use skill mix and role development to enhance job content and quality at all levels of the department. The department is supported by a robust management team, Pharmacy Technician and Pharmacy Support Worker structure that enables our Advanced Clinical Pharmacists to be actively involved in and focus on clinical and professional aspects of patient care. You will be responsible for managing and supervising the duties of Pharmacists, Pre-registration Pharmacists and ward support technicians within the clinical team. You will be responsible for supporting the medicines information services and training pharmacy staff to deliver these services. You will support the Clinical Pharmacy Team Manager in the delivery of the Pharmacy Vision and Values Strategy together with the other Clinical Pharmacy Team Leaders. The Trust has acknowledged the high quality of the services delivered by our clinical teams but we recognise the need for further integration of our pharmacists into clinical specialties to deliver a more strategic perspective of medicines optimisation and improve patient care.
