Primary Care Mental Health Integration (PCMHI) Provider Major DutiesThe following have been identified as essential elements of clinical duties for a CCC PCMHI Psychologist. The CCC PCMHI is expected to obtain and maintain mastery within the following domains of core competency developed by Robinson and Reiter (2007)

• Maintains daily opening access slots by alternating scheduled with open appointments and/or full open access scheduling.
• Applies principals of population based care as measured by monthly number of patient contacts, number of 30 minute appointments, number of encounters per patient, and range of interventions provided.

The Advanced MSA primary duties are to perform a variety of administrative support functions in connection with Veteran's inpatient and outpatient visits, care, and treatment while working collaboratively in an interdisciplinary coordinated care delivery model. Work involves specialized administrative judgment and the flexible use of a wide range of clinical flow processes relating to access to care across multiple clinics and specialties. Some of the major duties of this position include, and are not limited to:

• Schedule appointments in a timely manner. All appointments will be made with the patient's input, either in person or by phone. This may require a high level of coordination to avoid patients and sending sites not being able to conduct their CVT appointments.
• Daily review of active/pending consults, Recall list, and communications for accuracy and disposition.

The incumbent is assigned to VISN 4, within the Caregiver Support Program (CSP). The incumbent although stationed at the VISN-with primary role of CSP clinical eligibility reviewer-may have clinical privileges and credentialing at a VA Medical Center (VAMC) or Community-based Outpatient Clinic (CBOC). The incumbent may have up to 20% of their time dedicated to a combination of direct clinical care, academic teaching/training, research, and/or program evaluation at their home VAMC. This work would be separate from caregiver support. The duties for the incumbent, must be at least 75% administrative and related to the CEAT. The other duties will be determined by the incumbent and their VISN leadership.

Each individual CEAT team member will review the Veteran's medical record summary, most recent History & Physical, findings of recent testing at the facility, caregiver assessment and care team input. The team member will present their respective findings to the inter-professional team. The team will meet and render a multi-disciplinary decision regarding clinical eligibility, based on all eligibility requirements. If deemed eligible, the team will also determine the appropriate level of caregiving required (tier level). Each team will document the findings and determination and communicate the decision with the referring facility or region.

Major duties include, but may not be limited to:

• Reviews dispositions of PCAFC initial eligibility decisions; reassessments; and appeal decisions for clinical eligibly determinations, within 45 days of receipt from facilities and other CSP VISN CEAT. This is done from a psychological perspective.
• Analyzes mental health data obtained from VHA electronic health record and collateral records for use in determining PCAFC clinical eligibility and documents findings in approved CSP PCAFC templated notes.
• Works with inter-professional team members at Regional, VISN and facility levels to ensure PCAFC eligibility determinations are delivered objectively with strong, consistent inter-rater agreement and with documentation and communication of outcomes in a direct and coordinated manner.

Psychology Program Manager (Quality Assurance Officer) works within the Veterans Crisis Line (VCL), Quality, Training and Risk Management. Responsibilities are complex in scope and have a nationwide impact including the ability to ensure implementation of an effective enterprise quality management system that includes monitoring and evaluating the quality of clinical and administrative processes throughout the Veterans Crisis Line.

Major Duties include, but are not limited to:

• Serves as a subject matter expert in the areas of quality management and enterprise risk management for the Veterans Crisis Line and is responsible to ensure the effective implementation of an enterprise quality management system.

The incumbent is accountable to the Primary Care Social Work Supervisor in carrying out operational functions described above. The incumbent has clinical involvement in patient care by providing clinical case consultation to patient/family/community inquiries about services or patient care issues provided by the VA. Incumbent demonstrates basic knowledge of medical and mental diagnoses, disabilities, and treatment procedures to include acute, chronic and traumatic illnesses/injuries; common medications and their effects/side/effects; and basic medical terminology.

Therapy- Social worker will provide the veterans and their caregivers with ongoing psychotherapy (group and/or individual) and/or supportive counseling when appropriate. Incumbent is able to demonstrate the ability to utilize the DSM-V to ensure appropriate treatment recommendations.

Referral to service providers - Throughout the course of treatment the social worker is the subject matter expert on VA and/or community resources. The social worker will collaborate with other service providers in reassessing the veteran's needs for VA and community resources and entitlements. The social worker is responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his/her surrogate decision-maker.

Resource Development - The social worker is responsible for developing a resource file of VA and community social service programs and will refer the veteran to needed services.

Advocacy - Social worker understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate for themselves, thus fostering a sense of independence and empowerment.

Crisis Intervention - Social worker is experienced in making rapid assessments and developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long term placements. Social worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis.

Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:30 p.m.

Duties of the Occupational Therapist (Clinical Specialist) GS-0631-13, may include but are not limited to:

• Serves as a subject matter expert and provide clinical guidance area to other Occupational Therapist (OT) practitioners and consultation to other health care practitioners and outside groups about their specialized OT scope of practice.
• Modalities offered may include, but are not limited to heartrate variability bio feedback, clinical hypnosis, guided imagery, meditation or mindfulness, tai chi, qigong and yoga.
• Knowledges of specialized CIH interventions and services while utilizing comprehensive assessments of functional and occupational roles with standardized and non-standardized tools, which include Occupations (activities of daily living, instrumental activities of daily living, education, work leisure, social participation), Client Factors (values, beliefs, spirituality, body functions and structures), Performances Skills (motor, process and social skills), Patterns (habits routines, rituals, roles) and Context and Environment (cultural, personal, physical, social, temporal, virtual).
• Understands the theories of treatment in OT and their proper application through activity analysis, behavioral intervention, frames of reference and therapeutic procedures.
• Provides role modeling, guidance, and instruction to colleagues in best practices f or CIH.
• Consults other health care practitioners and outside groups about OT scope of practice as it pertains to CIH approaches. They may lead, initiate, or support research protocols in relation to their scope of specialization.

The Clinical Documentation specialist will provide active concurrent/retrospective review, provide feedback, and educate clinical care providers to improve the documentation of all conditions, treatments, and care plans within the health record to accurately reflect the condition of the patient and promote patient care. In addition, documentation should reflect documentation associated with MS-DRG assignment, case mix index, severity of illness, risk of mortality, physician profiling, hospital profiling, and reimbursement rules.

As a Clinical Documentation Specialist, incumbent is responsible for providing ongoing education to our providers and coding team members by conducting initial and extended-stay concurrent review on selected admissions and document findings identifying missed opportunities and clinical documentation improvements which will both enhance the quality of the health record documentation, maximize VERA and 3rd Party Collections. Incumbent identifies co-morbidities and complications and documents appropriately. Incumbent queries the medical staff and other clinical caregivers as necessary via written/verbal communication to obtain accurate and complete documentation. Provides ongoing education to physicians and other clinical care providers, related to documentation, changes in coding, compliance issues, profiling concerns, and reimbursement changes according to VA regulations and policies.
Orients and instructs new personnel and/or students from affiliated health information or medical record technology programs, at the direction of the supervisor, on unit operations, coding, abstracting, and use of an electronic medical record.
Advise and assist the Health Information Management section in the identification of training, in-services or educational needs of personnel. In addition, Clinical Documentation Specialist will present educational resources to the health information management staff and the professional staff as needed or provide goals as requested.
Works within a team environment; supports peers in meeting goals and deadlines; flexible and handles multiple tasks; works under pressure; and copes with frequently changing projects and deadlines.
Conducts re-reviews of codes abstracted for patient encounters identified by the VISN VERA Committee to determine if based on the documentation the specific VERA coding requirements were followed; corrects coding as needed to ensure proper patient classification in the VERA program.
