Primary Care Mental Health Integration (PCMHI) Provider Major DutiesThe following have been identified as essential elements of clinical duties for a CCC PCMHI Psychologist. The CCC PCMHI is expected to obtain and maintain mastery within the following domains of core competency developed by Robinson and Reiter (2007)
Clinical Practice Skills: - Maintains daily opening access slots by alternating scheduled with open appointments and/or full open access scheduling.
- Applies principals of population based care as measured by monthly number of patient contacts, number of 30 minute appointments, number of encounters per patient, and range of interventions provided.
- Integrates with primary care team and works closely in collaboration with PACT team.
- Provides education to PACT on BH issues, as well as, consultation on tips for successful care management and patient facilitation, even when no direct patient contact.
- On average, 75% of all appointments are brief in nature - meaning 30 minutes or less (will vary by setting, should range from 60-100%).
- Interventions and documentation focus on functional assessment as reflected in brief, focused notes and number of contacts per patient.
- Uses time-limited, evidence based, skills based interventions, self-management/home-based practice and brief assessments routinely (e.g., PHQ-9, GAD-7, PCL-5, AUDIT-C).
- Uses intermittent visit strategy and flexible patient contact strategies.
- Triages appropriately and efficiently to specialty mental health services.
- Provides behavioral medicine interventions via individual and/or group format, including but not limited to: stepped care level 2 tobacco cessation, alcohol misuse, weight management, coping with chronic illness, stress management, sleep hygiene, and other interventions will be provided as clinically appropriate, based on patient need.
Documentation Skills: - Writes clear concise notes that follow the 5 A's Template and utilize the PCMHI Note Titles in CPRS and complete relevant clinical reminders and CSSR-S and CSRE when applicable.
- Notes document curbside consultation results and notes are completed within 24 hours.
- Routinely provides feedback (verbal and/or written) to PCP and other PACT members same day as referral.
- Routinely Consults with PCPs and other PACT team members about plan of care for patients.
- Maintains at least 10% or higher on PACT21 - Percent of Primary Care Patients Engaged in PCMHI.
- Conducts effective curbside consultations.
- Integrates within primary care team and works closely in collaboration with PACT team
- Attends PACT meetings and daily huddles.
- Provides education to PACT on BH issues as well as consultation on tips for successful care management and patient facilitation, even when no direct patient contact.

Telework: Ad Hoc Telework (Limited) - Telework that occurs on an occasional, non-routine basis. Telework that occurs to complete short-term special assignments or to accommodate special circumstances even though the telework arrangement may occur continuously for a limited and specific period of time. Telework permissible to meet agency needs.
