To support and assist the Pharmacy Homecare Service Manager and Medicines Effectiveness Pharmacist in cost effective procurement and delivery of Homecare medicines and services for the OUH NHS Foundation Trust (OUH). To develop and support the pharmacy Homecare service at the OUH to develop services for the safe and timely supply of medicines to patients via third party homecare companies by establishing robust methods of managing the administrative work, monitoring homecare providers performance. Co-ordinate and communicate with clinical pharmacists integrated into service to implement and manage existing homecare services.

Due to new funding from the provider collaborative we are seeking to recruit a senior Occupational Therapist with Mental Health experience to work as part of a multi-disciplinary team providing Occupational Therapy interventions and working closely with the patients, their families, multi-disciplinary colleagues and external agencies (Prison, Police, MAPA Housing Provider, Probation). This also includes working with patients transitioning from inpatient services into the community. You will use standardised Occupational Therapy assessments to identify needs and to plan and implement individualised Occupational Therapy treatment interventions. Including co facilitation of Psycho educational groups/sessions, contribution to robust risk assessment and provision of Occupational Therapy formulations.
