Specialty Doctor/CESR in Emergency Medicine

We are excited to offer the following opportunity for doctors wishing to join our well established CESR rotation at the Royal Derby Hospital (UHDB).

You will commence with a fixed term Specialty Doctor contract in Emergency Medicine for 12 months and providing satisfactory progress is made at the end of the first year, successful candidates will have the option to progress onto the CESR programme.

Following successful progression through the programme, we will be able to guarantee specific secondment-style modules in the CESR programme - secondments offered include Anaesthetics, ITU, Acute Medicine and Paediatric Emergency Medicine.

In addition you will be allocated a designated Consultant supervisor for the duration of the rotation, as well as a CESR mentor.

You will be expected to participate in the 24-hour Emergency Department Middle Grade rota.

The post holder must be registered with the GMC and hold a full licence to practice.

Main duties of the role include:-

*Participation in the ED 24/7 middle grade rota.

*The ability to cover the whole range of clinical responsibility for both those patients seen in the Emergency Department and inpatients on our observation ward.

*To gain competencies over time with support and development to be able to manage the ED overnight.

*Clinical duties will include taking a lead role in the management of acutely ill and trauma patients. The qualities of leadership required as head of an emergency team are stressed and it is anticipated that these qualities will be gradually learned through the process of supervision of more junior doctors and involvement in their education, both formal and informal.

*To prioritise and time-manage effectively according to the activity of the department and to utilise appropriately the available senior staff for advice.

*Education - to be fully involved in supporting the training of all levels of staff: medical; nursing; para-medical.

*Quality improvement - you will be expected to assume responsibility (under the supervision of a Consultant) for a specific audit topic or quality improvement project.

The Emergency Department at the Royal Derby Hospital provides a 24-hour emergency service to a population in excess of 600,000. There is a separate Children’s ED, which is co-located with the Adult ED.

There is a six-bedded adult emergency observation/treatment ward within the Adult ED to facilitate overnight observation of appropriate patients. There is also a GP unit co-located next to the department.

A peer review by the College of Emergency Medicine praised the department, saying they were impressed with “the dedication of the (Consultant) team”, and “their desire to remain at the forefront of emergency care”. In addition, the ED at the Royal Derby Hospital won the award for ‘RCEM Clinical Team of the Year’ in 2017.

The Derby ED continues to enjoy a very strong reputation for teaching and training of junior doctors and places an emphasis on extended Consultant shop-floor presence and supervision.

The Consultant body has current FRCEM examiners, as well as experience at the CESR application process.

We run regular WBA clinics to allow dedicated time for training and assessment of all departmental clinical staff.

In addition, we have an active Research team, REMEDY, based in the ED and a strong focus on Quality Improvement in the department, with the use of SAFECARE quality improvement standards, to evaluate and drive quality patient care.

JOB SUMMARY The post holder will work as part of the memory service and provide high quality clinical assessments for people experiencing memory problems, including provision of a formal diagnosis of dementia where appropriate. Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines Negotiate with and motivate clients when working with care plans and treatment processesparticularly to socially excluded and vulnerable people Provide comprehensive mental health and social care assessments and provide care, treatment and educational interventions within the bounds of the agreed protocol and/or pathway within the service. Review the efficacy of anti-dementia medication that has been prescribed for service users in line with the service pathway & protocol. Act as care co-ordinator for service users as required. Attend multidisciplinary meetings and care programme meetings as required. Ensure that carers needs are assessed and appropriate intervention or onward referral is provided depending on need. 1.2 Working as part of a team: Recognise the limits of their own expertise and (in line with the local protocol/ pathway for assessment of suspected dementia), request additional assessments from more senior multi-professional colleagues when required in the context of complex case presentations or circumstances of diagnostic uncertainty. Access regular caseload and clinical supervision (as identified within the local service protocol/pathway), to summarise, present and discuss diagnostic assessments that have been completed with a senior medical practitioner within the service and/ or nurse consultant (where appropriate). Utilise an appropriate range of multi-agency and inter-professional resources in their work and pro-actively develop new partnerships. Work collaboratively in partnership with service users and health & social care professionals to provide specific interventions which are evidence based (including consensus best practice), which are most appropriate with regard to context and culture. Provide advice and support to other clinicians acting as a source of expertise Work in close collaboration with medical personnel regarding prescribing protocols, policies etc. Ensure that the patients GP is notified of any changes to that patients prescription, including commencement of any new medication in accordance with Trust policy. In the event of adverse drug reaction, to be accountable for ensuring that the GP/Consultant is advised accordingly. JOB SUMMARY The post holder will be part of a cohesive and enthusiastic multi-disciplinary team. You will be involved in providing specialist occupational therapy assessment and treatment for individuals receiving in-patient care enabling service users to engage in a wide range of individual and group based treatment activities. You will be actively involved in the promotion of occupational therapy within the service area. The post holder will be part of a well recognised service that places emphasis of staff support and professional development and you will be expected to participate in staff training and supervision. KEY RESULT AREAS: Clinical 1. To manage an individual caseload, providing Occupational Therapy assessments for service users receiving in-patient care and within the community, prioritising clinical needs and providing an effective service. 2. To work with service users to identify Occupational Therapy goals as part of the overall care plan, using occupational therapy assessment tools based on current evidence based best practice. 3. To plan and implement a client centred Occupational Therapy treatment programme on an individual and/or group basis, using graded activity to achieve therapeutic goals. 4. To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. 5. To demonstrate a broad level of understanding of the effect of disability and provide training and advice on lifestyles changes and adaptations to the clients social and physical environment. 6.
