Roles and Responsibilities:

• To work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker for a defined group of residents.
• To provide clinical leadership to a designated nursing team and ensure the provision of high standards of quality & care.

The Senior Social Worker for the CBOC provides advanced clinical social work services to Veterans and families in the Primary Care outpatient clinic setting. Social work services provided include psychosocial screening and assessment, psychosocial treatment and intervention, specialized case management and care coordination, discharge and/or after-care planning, health education and advanced health care planning, supportive counseling and caregiver support, and administrative and professional duties. The incumbent works closely with other members of the interdisciplinary treatment teams.
