The incumbent of this position must use a high level of skill in assessing and treating the complicated psychosocial problems of veterans with complex physical and mental symptoms and life-limiting illnesses. Referrals typically originate from other members of the interdisciplinary team and are provided in the outpatient clinic via telephone or video contact or face-to-face visits. Services are provided both to qualified veteran patients as well as their families, medical surrogates, and caregivers.

The incumbent's duties include but are not limited to completion of thorough biopsychosocial assessments, providing case-management for patients with ongoing palliative needs, supportive counseling for patients and their families, conducting goals-of-care discussions, acting as liaison with community hospice partners, providing counseling and assistance with Advance Healthcare Directives, and palliative program development.

• Provides continuity of care between Homeless Programs through services to Veterans with SUDs as they transition from programs such as Domiciliary or Residential Rehabilitation and Treatment Programs (RRTPs), Health Care for Homeless Veterans Contracted Housing, and Grant Per Diem Programs to HUD-VASH.
• Provides on-going substance use case management and early treatment interventions to promote sustained stability and abstinence in HUD-VASH subsidized permanent housing.

• Provide in depth clinical and economic support to payers, IDNs, and formulary committees, providing feedback to key internal stakeholders on customer trends in interpreting and using value evidence, reimbursement, and payer policy. May serve as main medical point of contact at prioritized local payer, regional payer, and IDN accounts

• Respond to unsolicited inquiries consistent with the MSL compliance standards

Responsibilities

The Case Manager supports patient access to therapy through Reimbursement Support Services in accordance with the program business rules and HIPAA regulations. This position is responsible for guiding the patient through the various process steps of their patient journey to therapy. These steps include patient referral intake, investigating all patient health insurance benefits, and proactively following up with various partners including the insurance payers, specialty pharmacies, support organizations, and the patient/physician to facilitate coverage and delivery of product in a timely manner.

• Investigate and resolve patient/physician inquiries and concerns in a timely manner
• Mediate effective resolution for complex payer/pharmacy issues toward a positive outcome to de-escalate
• Proactive follow-up with various contacts to ensure patient access to therapy
• Demonstrate superior customer support talents
• Prioritize multiple, concurrent assignments and work with a sense of urgency
• Must communicate clearly and effectively in both a written and verbal format

Qualifications

• Previous customer service experience, preferred
• High School diploma or equivalent, preferred
• Experience conducting and documenting patient health insurance benefit investigations, prior authorizations, and appeals, preferred (1-2 years)
• Knowledge of Medicare, Medicaid and Commercially insured payer common practices and policies, preferred
• Critical and creative thinking, preferred
• Knowledge of the Health Insurance Market Place and the Affordable Care Act, preferred
• Knowledge of Medical/Pharmacy billing and coding, preferred
• Strong attention to detail is vital
• Bilingual, preferred

TRAINING AND WORK SCHEDULES: Your new hire training will take place 8:00am-5:00pm CST, mandatory attendance is required.

Basic Qualifications:
