To undertake holistic needs assessment that explores individual patients and their carers / families physical, emotional, spiritual, practical, welfare rights and if appropriate the families pre and post bereavement needs, using a variety of tools to support the process. To be able to develop, update and monitor the individualised care plans ensuring they are patient centred and support the identified patient needs, including ongoing referrals to other services as required. To be able to deliver both generalist basic and complex specialist nursing care interventions that are responsive to the individual patients requirements. To be able to communicate highly sensitive information, ensuring patients and their families / carers are adequately supported. To be able to demonstrate through electronic and paper documentation and verbal communication the care undertaken and patient outcomes. To be responsible in leading on initial holistic assessments and reviews of care when required and discharge planning from service. To be able to communicate effectively to own team, extended team and outside agencies, ensuring continuity of care is maintained across the different care settings / home. To follow all agreed clinical procedures and statutory regulations related to medicine management. On identified shifts be responsible for dealing with any service enquiries, calls and referrals as required. To signpost and inform patients and their families / carers of the range of services available to them, external resources and day to day information relating to the service. Attend relevant MDT meetings as appropriate.

You should be able to demonstrate good keyboard skills and be computer literate and have experience of working in a reception or clerical environment. Previous Laboratory experience would be an advantage. If you are well organised, have excellent attention to detail, are a good communicator, team player, conscientious and flexible this job may suit you. You must be able to ask questions and take on board information given. You must be able to work on a 24/7 shift rota which includes dayshifts, late shifts, nightshifts, weekend dayshifts and nightshifts, Bank Holiday dayshifts and nightshifts including Christmas Day. This is a requirement of the post. Dayshifts and late shifts are 7.5 or 8 hours. Nightshifts and some weekends and Bank Holidays are 12 hours. On weekends, bank holidays and nightshifts you will often be the sole Pathology Reception worker on site (there will be Biomedical Scientists on site also).

To be responsible for those patients allocated daily. To ensure that excellent standards of support and clinical practice and care are achieved. Deliver specialist palliative nursing care to patients requiring planned and unplanned care.
