Therefore reducing the incidence of re-admission through education seeking early intervention providing an evidence based approach to clinical practice to improved patient care.

ICRAS aims to prevent inappropriate and/or avoidable hospital admissions by providing short term, intensive, multidisciplinary and holistic assessment treatment of care, ensuring collaborative working with existing social care providers and other agencies.

We wish to recruit nurses who have a clear understanding of developing the role of a nurse who can assess patients within a community. We work predominantly but not exclusively with older people. Our focus is ensuring patients health needs, often arisen from multiple co-morbidities and frailty, are met with the appropriate level of care. We work alongside the MDT to gain the best patient outcomes and facilitate patients to their preferred place of care, avoiding delay where possible.
