In line with the NHS Long Term plan, the principal aim of this role is to improve the quality of care for patients treated for atrial fibrillation (AF) and atherosclerotic cardiovascular disease (ASCVD) within the Coventry and Warwickshire ICB. The post-holder will be expected to provide a cost-effective clinical pharmacy service, and review patients within primary care who are not optimised for the above conditions.

The post-holder will work with secondary care colleagues, to help identify patients who are not optimised on their lipid lowering therapy in line with local guidance. They will also be tasked with reviewing patients who have a diagnosis of non-valvular atrial fibrillation (NVAF) in primary care and optimising their anticoagulation regimen if appropriate.

The role will involve data collection to demonstrate benefits to patient population in completing these interventions, as well as working closely with other health care professionals to provide teaching and education sessions.
• Work as part of the multidisciplinary team (MDT) within the primary care to develop a tailored lipid management plan for identified patients with ASCVD.
• Identification of sub-optimally treated ASCVD patients who are not achieving target lipid levels as outlined in in local guidelines
• Identification of patients with NVAF who require anticoagulation, and initiating therapy where appropriate.
• Review of treatment options for lipid management and anticoagulation in appropriate patient populations, in collaboration with patients
• Counsel and optimise cardiovascular medications in patients where appropriate

Due to internal promotion, an exciting and rare opportunity has arisen for an enthusiastic clinical pharmacist to join our friendly pharmacy team at University Hospitals Coventry and Warwickshire NHS Trust.The successful pharmacist willbe part of our highly valued and skilled Oncology/ Haematology and Palliative Care team and play a vital role in providing a specialised clinical service to patients.

Our welcoming pharmacy department has an innovative approach to pharmaceutical care with a ward-based technician service and pharmacist led clinics. We have strong links with the Medical School at Warwick University and De Montfort University School of Pharmacy.

You will be based at University Hospital Coventry, a PFI hospital opened in 2006 which is situated just off the junction of the M6 and M69.

Although based in this large Acute Trust you will have an overarching role to support and develop the pharmaceutical support for patients requiring palliative care across the locality. You will work closely with medical, pharmacy and nursing teams both in the acute trusts and in the community to improve the pharmaceutical care of these patients. You will act as a key member of the multidisciplinary team thereby impacting on day to day prescribing to promote best and safe practice and reducing clinical risk. In addition you will join the Palliative Care team in the Acute Trust, to provide direct pharmaceutical support for patients within their care.

The primary role of this specialist post is to lead on the management and provision of a comprehensive, high quality pharmacy service to the patients requiring palliative care and end of life services across Coventry and Warwickshire. Based at University Hospital Coventry and Warwickshire NHS Trust (UHCW), the post holder will have an overarching role to support the care of these patients across all settings including the other acute trusts in the network, local hospices and community.

This position requires you to have a clinical diploma or equivalent, possess excellent communication skills and be able to work as a member of a multi-disciplinary team whilst being able to manage your time effectively.

The post holder will:

· Work closely with medical and nursing staff to improve the pharmaceutical care of patients requiring palliative care and end of life services.

· Work with and support the Myton Hospice Palliative care pharmacist based at UHCW.

· Support the Pharmacy team to improve knowledge of palliative care as a speciality and review patients referred to the Palliative Care team at UHCW.

· Be responsible for all aspects of Medicines Management within palliative care and end of life services at UHCW NHS Trust and have an overarching medicines management role within palliative care across Coventry and Warwickshire.

· Lead on the provision of education to healthcare professionals in the pharmaceutical aspects of palliative care and end of life services across the above care settings.

· Develop policies, guidelines, conduct audits and perform research relating to the use of medicines within palliative care and end of life services.

· Support the Quality, Innovation and Patient Safety aspects of Palliative Care at UHCW to ensure care is outstanding and continually improving.

Role-specific duties

1. Provide a comprehensive clinical pharmacy service for palliative care patients at UHCW NHS Trust, attending board rounds and multidisciplinary team meetings.



2. Support a comprehensive clinical pharmacy service for palliative and end of life care patients across Coventry and Warwickshire by acting as pharmacist representative on the C.A.S.T.L.E. (Care and Support Towards Life’s End) expert advisory group which represents clinical providers of palliative and end of life care across Coventry and Warwickshire.



3.Provide pharmacy representation and input into other relevant groups such as the ICS EOL working group, the West Midlands Specialist Palliative Care Audit and Guidelines Group (SPAGG) and the Central England Palliative Care Pharmacy Group.



4. Provide strategic guidance on clinical pharmacy issues relating to palliative and end of life care services and provide professional pharmacy leadership in this area of care



5.Proactively influence safe and cost-effective drug therapy with other senior clinical staff within palliative care and end of life services. This will include performing new drug evaluations, financial reporting and development of policies.



6.Identify areas where improvements in both quality and cost effectiveness of pharmaceutical care and prescribing may be encouraged, and influence others to incorporate this for the benefit of the wider health economy.



7.Work with senior staff in palliative and end of life care services to review policies and procedures to ensure safe and effective handling of medication. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and CQC Requirements. Undertake risk management and conduct audits relating to supply, storage and administration of medicines.



8. Lead, develop and deliver education and training in the pharmaceutical aspects of palliative care to other healthcare professionals. To encourage and cultivate an environment of clinical and practice-based research and audit. Role-specific duties To practice as an expert practitioner, providing advice and consultancy to patients, healthcare professionals and other agencies relating to pharmaceutical aspects of your clinical speciality within the department and on the wards/clinical areas. To participate in outpatient clinics as a non-medical prescriber (as required and funding permitting). Provide oral chemotherapy pre-treatment consultations (as required and funding permitting). To provide a highly specialist clinical pharmacy service within an allocated clinical area which may include adult oncology, adult haematology and paediatric cancer services. To operate as an authorised pharmacist within the Pharmacy Aseptic Unit, handling and final checking chemotherapy and other aseptically dispensed/ manufactured medicines. To participate in chemotherapy screening (clinical verification) of prescriptions as per department rota. To support all aspects of the cancer drug commissioning arrangements locally. This will include funded drugs and those available via a compassionate use or similar scheme. The RISE CYP Eating Disorders service aims to work with children and young people with a primary diagnosis of an eating disorder and their families/carers in order to restore both physical and psychological wellbeing. We offer a range of evidence-based treatments for eating disorders and deliver this on an outpatient basis across the Trust. This includes CBT-E, CBT-T and CBT for comorbidities such as Anxiety and Depression. In addition, the service is planned to expand, including the development of an ARFID pathway, and it is envisaged clinicians will be offering CBT-AR in due course.

· Providing supervision to designated CBT Therapists within the ED team.

· Providing high quality CBT assessment, formulation and intervention for children and young people presenting with complex mental health difficulties and co-morbidities.

· Providing CBT consultation to colleagues and professionals within and outside of the service.

· Identifying and delivering, together with the ED clinical lead, CPD relevant to CBT therapists. .

The post holder will: Provide specialist clinical pharmacy service to T&O inpatient and elective pathway, major/emergency trauma services which including MTECU and St Cross surgical oversight. Provide specialist clinical pharmacy service to all theatres, surgical and associated professional staff to improve the pharmaceutical care of patients. Be responsible for all aspects of Medicines Management and Optimisation within T&O and theatres. Working closely with T&O team, theatres, surgical specialties, critical care and associated professional teams to improve medicine management. To work closely with and the Intensive Care and Surgical Pharmacists as needed, having a good working knowledge of these areas. Lead on the provision of education to healthcare professionals in the pharmaceutical and medicines management aspects of their area. Support and lead on the development of service improvement project in their directorate for example pre-op clinics, day case pathways and development of community diagnostic centres. Be responsible for the development of policies, guidelines, audits, reports (both financial and clinical) and research relating to the use of medicines within their area. Be responsible for organising and being the lead for the day-to-day operational aspects of the pharmacy teams within their allocated areas. To be responsible for providing a cost-effective clinical pharmacy service, which is patient and medication orientated through effective supply of medicines, prescription monitoring and prescribing, which ensures the patient receives safe, effective and timely drug therapy. 1. Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic laboratory and when necessary take accurate drug histories to facilitate this. Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients safety and effective therapy2. Provide when necessary, through checking dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic laboratory and trials/Homecare services.3. Ensure patients discharge medication is clinically appropriate, accurate and when possible, dispensed in advance of the patients discharge. Reduce pharmaceutical waste by utilising patients own drugs in the discharge process.4. Provide information to patients concerning their medication and answer any resulting questions or queries.5. Work flexibly across all of the Pharmacy departments, covering the work of colleagues during peak periods or absence as requested by Clinical Leads or the Senior Management Team. 6. To participate in the competency based assessments and training of junior staff within the team. To ensure areas of concern in the clinical and operational performance of staff within the team are identified and reported to the Clinical Leads. To participate in performance management of members of the team as needed. 7. Participate in appropriate training and development activities including your own personal development and participate in the education and training of colleagues and all other healthcare professionals on medicine legislation, pharmacology, medication risk and best prescribing practice. 8. Contribute to local undergraduate and postgraduate education and training within your speciality, both within the Trust and for other programmes of study. 9. To work with the Lead Pharmacy Technician/Pharmacist for Education and Training to ensure the effective delivery, assessment and induction of staff in the department. 10. Act as a Co-ordinator for Keele University Clinical Pharmacy Diploma and mentor for other pharmacists within the Trust. 11.
