It takes a Village to care for our patients and as an IKC Medical Office Assistant (Care Coordinator/CC), you will be at the heart of all the action.  Our CCs work together as a team to facilitate the planning and organizing of all critical activities for our patients.  The patients we care for are some of the most complex patients and need an excellent care team to help manage their chronic conditions.  The Care Coordinator works closely with our Nurse Practitioners, RN Case Managers to make sure our patients receive the highest quality care.

Position Details:

• Hybrid - working on site 4 days per week, and remotely from home the remaining day(s).

• Full Time M-F schedule. Must be flexible.

• Commute within Lakewood, CA market visiting multiple Davita clinics in the area.

Responsibilities:

• Assist the clinical team with moving the patient through the continuum of care by communicating with and completing tasks as directed.

• Assist the care management staff with implementation and documentation of the discharge care plan that includes initiation of referral forms, gathering clinical information, initiating the process of completing a chart and appointment scheduling.

• Communicate directly with patients on a timely basis for all scheduling requirements.

• Maintain accurate and complete documentation of records. Forms are fully-completed, legible, have no unapproved abbreviations and have the proper signatures.

• Ensure that documents get to appropriate parties in a timely manner, without compromising confidentiality or patient privacy procedures.

• Demonstrate exceptional two-way communication using clear and well-organized oral and written techniques.

Qualifications of the Medical Office Assistant:

• High school diploma or GED required

• At least two (2) years of experience in a professional office environment

• Experience in a medical office or hospital setting a bonus

• A background in customer service is a plus

• Prior remote work experience is a plus

• Ability to work with and resolve confidential information and/or issues using discretion and judgment

• Excellent verbal communication skills over the phone

• Proficiency with Microsoft Office/Excel/Outlook

Job Description

The Certified Medical Assistant is responsible for assisting in the coordination and support of the physicians and/or other medical teams in the Cancer Center. This includes rooming patients and obtaining vital signs, basic screening and collecting information for medication reconciliation, assisting with procedures, scheduling, calling offices and acquiring records, chart prep, and other duties as assigned.

QUALIFICATIONS
High School or equivalent.
Completion of Accredited Medical Assistant Program or documentation of training.
Current clinical experience in ambulatory care as a Certified Medical Assistant is strongly preferred but not required.
Excellent communication skills.
Computer skills including word document, email etiquette and use of EPIC preferred.

License/Certification

Major Duties

Oncology Formulary Management and Utilization:

• Manage the oncology formulary by evaluating and onboarding new agents.

• Determine the appropriate site of care for patients receiving oncology treatment, considering factors such as inpatient, outpatient, or regional campuses.

• Oversee the review process for off-label, non-formulary, or other non-standard use of antineoplastic agents.

• Promote appropriate, standardized and guideline-based antineoplastic utilization across all El Camino Health (ECH) sites where appropriate.

• Review provider requests for exception orders and evaluate for literature support to guide the prior authorization process when applicable.

• Collaborate with relevant stakeholders, including physicians and other healthcare professionals to ensure the safe and effective use of these agents.

Collaboration with Pharmacy Informatics Teams:

• Work closely with pharmacy informatics teams to contribute to the development and revision of Electronic Health Records (EHR) resources.

• Ensure that EHR resources support the safe and effective use of oncology medications and adhere to current guidelines and best practices.

Education:

• Coordinate, facilitate, and provide education, training, competency assessment and performance improvement to pharmacists, pharmacy residents and other healthcare professionals for the appropriate use of antineoplastics as well as on best practices and the latest developments in oncology pharmacy.

• Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.

Quality and Performance Improvement:

• Contribute to the institution’s antineoplastic stewardship program through compiling data and metrics on antineoplastic utilization in the outpatient and inpatient settings, reporting data and findings to relevant stakeholders.

• Monitor antineoplastic utilization practices to identify areas for improvement and implement necessary changes.

• Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.

• Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.

• Stay updated on the latest advancements in oncology pharmacy and ensure that the pharmacy services align with current standards of care.

• Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all ECH sites and promote and enhance patient outcomes through safe, effective, cost-effective, and appropriate medication therapy use for patients receiving antineoplastic therapy.

Qualifications – All

• Doctorate degree pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.

• A one (1) year accredited PGY1 (Post Graduate Year) pharmacy residency.

• A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with minimum of 2 years of oncology experience that includes pharmacy regulatory compliance, compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.

License/Certification/Registration Requirements/Credentialing

• Valid California state pharmacist license with no disciplinary action(s) against the licensee that prohibits the licensee from practicing pharmacy.

• Valid CA driver’s license

• Valid NPI (National Provider Identifier) number issued by the Center for Medicare and Medicaid Services (CMS)

Ages of Patients Served

This position is responsible for the assessment, treatment, and care of adolescent, adult, and geriatric patients.

Under limited supervision, provide administrative oversight for a service line leadership team, including the coordination of communications and pertinent data among a group of the directors’ subordinate leadership and oncology providers.  This is including but not limited to managing the director’s calendar and responding on behalf of the oncology leadership team and providers to internal and external stakeholders.

Performs and oversees special projects or assignments that are highly sensitive or complex, requiring a high level of independence and initiative in execution and implementation. Handles, details, and directs work of a highly confidential and critical nature to support the oncology leadership team on matters of significance. Provides planning for special events, includes managing vendors, scheduling, selecting venues, and coordinating details.   The administrative assistant will assume the key leadership position of coordinating all aspects of the Multidisciplinary weekly Breast Tumor Board and General Tumor Board at Mountain View Campus as well as the twice a month Los Gatos General Tumor Board.

• Directs the daily administrative operations for the Oncology Service Line Administrative offices.

• Develops and implements efficient and effective systems for managing information, projects, and communications to ensure accurate and timely responses.

• Functions as the gatekeeper, understanding the oncology leadership teams requirements, prioritizing issues according to urgency, and allocating time accordingly. Protects Service Line Directors time/schedule.

• Maintains a comprehensive list/database of initiatives and assignments, to include actions delegated to other offices.

• Responds to a wide variety of questions and issues from patients, staff, and the public on behalf of the Oncology Service Line Leadership team, including those highly sensitive in nature.

• Research, compiles, and summarizes complex and/or highly sensitive issues, policies and topics; provides background information, talking points and briefing materials for the Service Line Leadership Team.

• Briefs service line leadership in a timely fashion regarding priorities and deadlines on the status of pending issues.

• Organizes ad hoc committees appointed by the director on a broad range of issues.

• Prepares confidential reports and summarizes issues for senior staff, board members within El Camino Health.

• Handles the details of complex and highly sensitive matters on behalf of the Oncology Service Line Leadership.

• Prepares responses to urgent requests on behalf of the Oncology Service Line Leadership; takes charge of complex and highly sensitive matters, and independently investigates issues and problems.

• Makes high-level contacts of a sensitive nature with internal and external individuals, requiring a high level of discretion and diplomacy.

• Researches, analyzes, and synthesizes information to prepare Oncology Leadership Team briefings, reports, and summaries.

• Coordinates information from other departments and tracks to ensure completion with sufficient time for Oncology Service Line Leadership Team review in advance of meetings.

• Manage small to medium size/scope projects.

• Will assume the key leadership position of coordinating all aspects of the Multidisciplinary (Medical Oncologists, Radiation Oncologists, GYN Oncologists, Radiologists, Pathologists, Nursing, Social Work, Palliative Care, Pharmacists, Inpatient Nursing, Nurse Practitioners, Nutritionists, Tumor Registry Team) weekly Tumor Boards to assure compliance with the American College of Surgeons Commission on Cancer Standards.

Qualifications

• Associate degree with seven years of administrative experience, at least three at senior leadership level OR Bachelor's degree with five years' experience or equivalent.

• Prior healthcare administrative experience in a senior leadership setting preferred.

• Demonstrated experience managing multiple complex calendars for senior leaders.

• Strong interpersonal skills: ability to communicate and influence effectively with all levels of the organization including outside vendors, employees, physicians and members of the leadership team, both verbally and in writing.

• Strong customer service experience.

• Advanced experience using MS Office applications including Word, Excel, PowerPoint and Outlook.

PURPOSE AND SCOPE:

The registered professional nurse RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP).  This position is accountable and responsible for the provision and coordination of clinically competent care including assessment planning intervention and evaluation for an assigned group of patients. Assesses and manages patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols and communicates patient related issues to the physician as needed. As a member of the End Stage Kidney Disease (ESKD) health care team this position participates in decision-making teaching leadership functions and quality improvement activities that enhance patient care outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy procedures standards of nursing practice state and federal regulations.

• Performs all essential functions under the direction of the Supervisor with guidance from the Educator Preceptor or in collaboration with another Registered Nurse.
• Performs ongoing systematic collection and analysis of dialysis data for assigned patients and documents in the patient medical record makes adjustments or modifications to treatment plan as indicated and notifies Supervisor or physician as needed.
• Assesses collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family.
• Directs and provides in collaboration with the patient home care partner direct and ancillary patient care staff all aspects of the provision of safe and effective delivery of dialysis therapy to assigned patients.
• Administers medications as prescribed or in accordance with approved algorithm(s) and documents appropriate medical justification and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource for health care team participates in staff training and orientation of new staff as assigned.
• Ensures correct laboratory collection processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
• Identifies expected outcomes documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
• Ensures patient awareness related to transplant and treatment modality options.
• Assists in the identification evaluation selection and education of Dialysis candidates and Home Partners.
• Performs assessment and identifies barriers of the Dialysis candidate’s home environment and partner / family readiness and ability to perform dialysis treatments in the home. 
• Trains Home Dialysis patients and / or Home Partners on the safe effective operation and maintenance of all Dialysis equipment and treatment supplies through an organized and formal Dialysis Training Program.  
• Required to complete CAP requirements to advance.  
• Performs all other duties as assigned by Supervisor.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

Assigned oversight of LPNs/LVNs RNs Patient Care Technicians and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge after meeting all the following:

•  Successful completion of all FKC education and training requirements for new employees.
•  Must have a minimum of 12 months experience as a RN.
•  Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis.

EDUCATION and LICENSURE:

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification
• Must meet the practice requirements in the state in which he or she is employed.

EXPERIENCE AND REQUIRED SKILLS:

• Entry level for RNs with less than 1 year of Nephrology Nursing experience in the last two years as a RN.
• Minimum of 1-year experience as a Registered Nurse (preferred)
• Home dialysis therapy experience (preferred).
