Core Duties To provide specialised clinical pharmacy advice for allocated wards / teams including attending their multidisciplinary team meetings. To lead in the delivery of the clinical and patient services to allocated wards / teams, including leading on any medicines-related projects. To identify audit topics for allocated wards / teams. To contribute to and encourage pharmacy-based and multidisciplinary clinical audit. To support the work of the dispensary; dispensing and checking inpatient and outpatient prescriptions, as required by the Dispensary manager. To provide pharmaceutical advice to patients and staff of CHC Trust in the dispensary and by telephone as part of the pharmacy service. To liaise with medical and nursing staff in the provision of the Pharmaceutical Service to Combined Healthcare Trust Clinical Pharmacy Service To provide a specialised clinical service to allocated wards / teams including: - Provision of specialist advice to patients, relatives and other healthcare professionals Review of prescription charts according to Trust procedures. This involves making recommendations tailored to the needs of specific patients or situations. This includes checking medicine doses are appropriate; checking for interactions; addressing co-existing medical diseases and conditions such as hepatic and renal impairment. Endorsing of charts according to Trust procedures Risk Management and compliance with Medicines Legislation. To participate in the Care Plan approach. To participate in Consultant ward rounds and Multidisciplinary team meetings where appropriate. To provide cover for additional wards and units as required. To participate in the forward planning and development of the Clinical Pharmacy Service. To participate in the systems to measure clinical pharmacy input and provides such information on request. To investigate pharmaceutical problems arising from use of medicinal products and to participate in and encourage medical staff in incident reporting and Yellow Card completion. To support and advise all prescribers within the pharmacists areas of competence To work closely with and support the work of other pharmacy staff within the pharmacy service to ensure effective team working. Management Role To prepare and present regular evaluated medicines usage and expenditure reports (minimum quarterly) to specified wards / teams in liaison with the Principal Pharmacist (Clinical) To identify areas of potential cost savings in medicines expenditure and to advise and make recommendations to the team in managing their medicines budget in liaison with the Principal Pharmacist (Clinical) To attend and participate in pharmacy team meetings. To risk assess the Pharmaceutical Service for which s/he is responsible through the maintenance and development of safe systems of work To give advice on pharmaceutical matters to all members of the Healthcare professions, patients and carers to facilitate safe and economic use of medicines within services for Mental Health, including counselling on the use of aids to concordance, and to promote patient choice. We are looking for a Nurse who can demonstrate experience in delivering care for the acutely ill adult. The Surgical Nurse Practitioner team provides support for the patients within the Surgical Division and is first point of contact for all acutely ill patients out of hours as part of the Hospital at night team. You must have a positive attitude with well-developed interpersonal skills, leadership, vision and drive to fulfill this key organizational role. Successful candidates will be able to work autonomously and manage their own work load.

1. Clinical 1.1 To act as a Specialist Nurse for patients undergoing bariatric surgery and to have direct clinical involvement in complex patient care in the outpatient and in-patient setting. 1.2 To provide a nurse led service for those patients requiring follow-up care after surgical intervention. 1.3 To facilitate all investigations for those patients suspected of post-surgical complications. 1.4 To facilitate the bariatric surgery telephone support service. To be available for patients and their carers in order to assess and manage day to day problems and anxieties and to refer on where appropriate. 1.5 To co-ordinate the bariatric surgery journey, acting as the key worker for those patients under your Care on the Bariatric pathway. 1.6 To assess patients and their complex needs and those of their families and plan, implement and evaluate specialist programs of care, communicating with those responsible for the care, and documenting the appropriate details whilst at all times maintaining confidentiality. 1.7 To refer patients for diagnostic investigations with suspected or proven, endocrinal diseases. 1.8 To ensure that specialist bariatric surgery care information is available. Provide highly specialist advice and support to patients with obesity problems which will often include communicating highly sensitive information about diagnosis, treatment options, and entry into clinical trials, prognosis and outcome of illness including future care. 1.9 To offer highly specialist emotional, psychological and practical support and symptom control to the patient and their family/carer throughout their bariatric surgery pathway. 1.10 To ensure that all practice conforms to current written protocols and policies of UHNS and be involved in development of new policies. 1.11 To ensure consumer views are sought by facilitating patient involvement in the development of services. 1.12 To have phlebotomy skills. 2. Educational and Development 2.1 To lead on specific areas of bariatric surgery and identify areas where current practice needs to be reviewed and to make proposals for new initiatives. 2.2 Provide the education of pre and post registration health professionals in formal and informal settings. 2.3 To be available as an expert resource for UHNS staff. 2.4 To be involved in specialist internal teaching for non-professionals, patients/carers. 2.5 To be involved in the development of specialist information booklets and leaflets. 2.6 To help maintain and further develop a UHNS multi-professional bariatric surgery team. 2.7 To identify areas of care that could be researched to lead to improvements in care. 2.8 To regularly participate in nursing and multi-professional research projects and lead clinical audit within the specialty of bariatric surgery nursing within the Trusts clinical governance and research programme framework. 2.9 To disseminate current research findings to professional colleagues through publication and presentations. 2.10 To receive regular clinical supervision, if post holder wishes, and provide clinical supervision for other staff and students. 2.11 Take every reasonable opportunity to maintain and improve professional knowledge and competence. 2.12 To partake in a 6-12 month period of ongoing training and development of the individuals specialist knowledge of bariatric surgery. 2.13 To participate in personal objective setting and review, including the creation of a personal development plan at annual appraisal. 2.14 In the context of the individual's own knowledge, experience and sphere of authority, assist peers and subordinates to develop professional competence in accordance with their needs and the Divisional objectives. 3. Professional 3.1 To be a key member of the bariatric surgery team contributing to patient assessment and facilitating a collaborative approach to care planning at weekly bariatric surgery Multi-Disciplinary Team (MDT) meetings and contribute to all other MDT meetings, i.e. MDT away days, Network Site Specific Group. 3.2 Ensure that nursing practice is in keeping with NMC Code of Professional Conduct and standards and reflects policies and procedures of UHNS. 3.3 To ensure that bariatric surgery care nursing interventions are evidence based, or best practice and that a uniform high standard of patient care is provided. 3.4 Ensure that plans for the service link to the goals and priorities of the Trust/Directorate and to the bariatric surgery Action Plan, following Peer Review. 3.6 Develop care pathways for bariatric surgery patients. 3.7 Keep up to date with professional issues and ensure they are reflected in practice. 4. Management 4.1 To assist in development of specialist bariatric care policies, procedures, protocols, standards, guidelines and pathways within UNHS. 4.2 To collect and present statistical information and reports as requested, to the Trust. 4.3 To maintain adequate confidential records of all patients and ensure that all relevant information is documented on the patient's medical records/pathway. 4.4 To contribute to the Divisions Clinical Governance framework ensuring that all aspects of the service provided is to the highest clinical standard. 4.5 To monitor practice in relation to Bariatric surgery and assist in the management of complaints. 4.6 To manage time effectively to meet service objectives. 4.7 7 To manage a system of data entry on to the National Bariatric Surgical Register under the supervision of the lead bariatric Clinical Nurse Specialist. 5. Health and Safety: 5.1 To take reasonable care for their own Health and Safety and that of any other person who may be affected by their acts or omissions at work. 5.2 To co-operate with University Hospital of North Staffordshire NHS Trust in ensuring that statutory regulations, codes of practice, local policies and departmental health and safety rules are adhered to. 6.
