The Rehabilitation Assistant (RA) will deliver rehab to a group of patients under the guidance and supervision of a registered allied health professional in the Maximising independence team.

The Maximising Independence team aims to provide a high quality, responsive rehabilitation service helping adults to regain their independence following illness or injury, to support them with their Long-Term conditions and to promote health and wellbeing

The team comprises of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Senior Rehab Assistants and admin Assistants.

The team is based at Battersea Studios in Wandsworth and offers home based sessions to patients in the Wandsworth locality.

To work as part of a multidisciplinary therapy team delivering a community based rehabilitation service to patients with a variety of conditions under the supervision of qualified therapists.

Post Covid Assistant Practitioner - Band 4 - Battersea Studios, SW8

An exciting new position for an assistant practitioner has arisen to join the South West Post Covid Assessment Service, covering Wandsworth and Merton, working alongside a multidisciplinary team to deliver holistic and coordinated care to patients. The post-holder will join the team based at Battersea Studios (SW8), to support community patients with post-covid-19 syndrome. You will be supported by highly specialist physiotherapist and occupational therapists in your clinical and professional development.

The team has established links with IAPT, community providers as well as community and voluntary sector resources. Prior experience is not required; we are looking for candidates who are enthusiastic, flexible and a have desire to learn. The post-holder will be supported to gain the skills and confidence to assist in patient triage and assessment. In conjunction with the team, they will help coordinate care, case manage selected patients and provide specialist treatment.

As an Assistant Practitioner within the Post-Covid Assessment Service you will get the chance to work with and under guidance of a qualified Allied Health Professional (AHP) supporting service users, their families, friends and carers. You will undoubtedly play an important role in how they experience our service.

You may often work independently to follow treatment plans or provide support in using the Living with Covid app as well as other digital technology - so if you have good IT skills this could be the job for you! The Council will pay for the PVG membership cost initially, either £18 for existing PVG scheme members, or £59 for non-members, and this cost will be deducted from an employee’s first FULL month’s salary payment.

To provide financial support to the Finance Controls team staff in relation to the a
range of tasks including community care commitments, billing, reconciliations, finance processing and payment of allowances. Liaising with managers, suppliers and elected Members and resolving various issues whilst providing a high performing, customer focused finance support service to the services and on behalf of the City Council and external bodies

The post holder needs to hold as a minimum:

• Standard Grades or equivalent

The post holder will spend their the time providing patient facing or related care, working within a multidisciplinary respiratory team to deliver community respiratory service in South West division, providing specialist services including hospital at home, home oxygen assessment & review service, clinics, stable disease management and home visits toprovide expert specialist care to patients with chronic respiratory diseases.
