A great opportunity has arisen to join Warrington Recovery Team. We are looking for an experienced and dedicated B7 practitioner to support our RESP Pathway, in the role of Personality Disorder Link Worker.

In this role, you will work closely with the wider Recovery MDT and Psychology, offering support and psychological based interventions under NICE guidance, to clients with a diagnosis of Personality Disorder.

You will work closely with experienced colleagues in the MDT, with ongoing support and clinical supervision. You will also have the opportunity for continued professional development via access to specific, in depth training to support you in this role.

Warrington Recovery Team are a well led, highly regarded service. Our base is Wakefield House and our core hours are Mon to Fri 9-5 (subject to change)

You will undertake assessments and offer specific psychological based interventions to clients under the RESP Pathway in Team.

This will involve working with clients, both on an individualised 1-1 basis and group work.

You will work closely with the Recovery MDT and wider services, to promote recovery and achieve best outcomes for clients under the RESP Pathway.

You will take a lead role in identifying and formulating service users needs and working together with other professionals and agencies, to help meet those needs, providing specific goal based interventions and support.

Mental Health Care

These clinical divisions provide mental health, brain injury, addiction and child and adolescent mental health services in different areas across Halton, Knowsley, Liverpool, Sefton, St Helens and Warrington. Secure Care

This clinical division is responsible for providing specialised mental health and learning disability services in low, medium and high secure settings for service users with a primary classification of mental illness, and personality disorders. Byron ward is an assessment and treatment facility for adults with learning disability and or autism who require specialist support. · Working as part of a MDT to assist in the management of clinical trial patients, maintaining effective communication with patients, carers & professionals to ensure high quality service delivery.

· Managing & overseeing a portfolio of research studies across various sites.

· Training & assessing research staff as competent in line with the research competencies framework, acting as a resource and role model for all aspects of research clinical practice to optimise patient care and clinical practice.

· Use relevant clinical knowledge to identify patients suitable for clinical research using inclusion and exclusion criteria and utilising patient records, visiting clinics and wards and outpatients.

· Carry out physical assessments, take blood/urine samples and process according to specific research protocol.

· Demonstrate a comprehensive understanding of treatment options, treatment side effects & disease processes to support patients in making an informed choice. Providing ongoing information, education & support to patients regarding clinical studies & specific trial treatments & procedures.

· Provide day to day supervision for band 5 research nurses/ research officers within the research team.

· Implementing quality systems to ensure that the delivery of studies meet requirements with regards to relevant frameworks & participate in Good Clinical Practice training, keeping up to date with any changes in legislation or practice.

Warrington CYPMHS are looking to appoint a Band 5 core or none core registered practitioner to their well established high performing team. The team is multi-disciplinary and made up therapists, nurses, social workers, psychiatry and support staff. We are looking for a highly motived, enthusiastic individual who has a passion for working with young people in the community. Core tasks include mental health assessments and care co-ordination.

As a Mental Health Practitioner, the post holder will provide high standards of evidence-based client centred care to children and young people with mental health [CYPMH] problems within the THRIVE Framework for service delivery.

The post holder will be expected to provide interventions to children and young people and their families/carers referred into the service.

CLINICAL

To hold a caseload of assessments, treatment upon a conceptual framework and employing research based evidence.

To provide direct therapeutic interventions in line with the grading of the post, including assessments, formulation and therapeutic input into children, young people and families referred into the service.

Undertake risk assessments and care plans accordingly, in line with the THRIVE framework

To work in ways that are sensitive to the needs of children and young people and families from a wide range of racial, cultural and religious backgrounds.

To participate directly in the teams duty rota’s in line with the grading of the post.

To exercise responsibility within supervisory, managerial and accountability structures, for the assessment, treatment and discharge of clients

To produce a clinical formulation and intervention plan founded on evidence based practice in cooperation with the child young person and carers

To hold responsibility for standards of clinical practice within area of work.

To work jointly and collaboratively with other team members in order to enhance and develop the provision of services to young people and their families. To have an effective knowledge base of other modalities so as to engage and work effectively with colleagues in other disciplines.

Undertake risk management for all children including the protective and risk factors present in the network of significant relationships.

Identify and respond appropriately to child protection issues, as described with the guidelines of Local Safeguarding Children’s Board and the Trust safeguarding policies

Maintain clear and comprehensive electronic clinical records, to ensure information is accurate and complies with Data Protection Act and professional body and Trust standards.

To engage with using routine outcome measures as clinically appropriate including their use in clinical practice and inputting data into clinical records.

To work collaboratively with children, young people and their families and contribute to the trust’s service user participation agenda.

What will I do?

CLINICAL:

Provide expert clinical practice to clients whom pose significant challenges/risk to themselves or others with associated problems e.g:

Challenging behaviour

Mental health issues

Complex communication needs

Profound and multiple disabilities (including unstable/complex epilepsy)

Forensic behaviour



As an autonomous independent practitioner assume continued responsibility for caseload and clinically support interventions to clients with complex challenging behaviour and/or forensic behaviours who may be at risk of placement breakdown and/or hospital admission.

To take the lead role in facilitating/co-ordinating access to primary and secondary health care services for people with learning disabilities. Developing/influencing and implementing changes at both local and trust wide level.

Provide clinical specialist practice in relation to service users with complex needs who may display forensic/risk behaviours, undertaking complex assessments, formulations and risk management plans in conjunction with the service user, their families/carers and other professionals.

Responsible for the assessment of need, care planning and care management of clients and carers in accordance with the Trust Effective Care Co-ordination policy within Mental Health Services.

Provide training when required to a wide range of people internal and external (health, social services, voluntary/private sector staff, clients and carers).

Contribute to the strategic development of services to meet the Valuing People agenda through effective facilitation and leadership.

Ensure links to professional leads are developed and maintained

Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.

Work in a professional and collaborative manner with the multidisciplinary team.

Participate and promote positive intra-agency working.

Set standards of service user care based on sound research findings.

Where indicated and following completion of the relevant training the practitioner will act as a non-medical prescriber, at all times working within their sphere of clinical competence.

Ensure effective deployment of staff and other resources within the Intensive Support Team to deliver the required service and agreed standards.
