Here at Croydon we put the patient at the heart of everything we do, as we strive to deliver and develop our modern and comprehensive Elderly Care service. We are aiming to recruit a geriatrician to join our existing team of fourteen consultants working across our acute and community services. The successful applicant would be responsible for providing in-patient care for patients living with frailty and presenting with frailty syndromes, as well as contributing to our developing Acute Care of the Elderly service. As such, they would be working on our Frailty SDEC and providing cover for the Front Door Frailty team (“Team ALFA”) on a rotational basis.

There is commitment for night and weekend on call for acute admissions. The Trust divides the take according to age based criteria such that the post holder would be responsible for admissions for those over the age of 80 years (and over 65 years living in a care home). Weekday on calls are one day a fortnight and weekends at a frequency of 1 in 12.

Administration and Management of Resources:
• To receive and triage a range of enquiries from professionals, patients, and prison staff, consult with internal and external sources to resolve queries, problems, or complaints. Enquiries may be of a highly sensitive, contentious, or distressing nature requiring judgement, tact, and sensitivity to reach an appropriate resolution.
• Maintain an effective message taking system ensuring that all messages are passed on to the appropriate individual in a timely manner, in some situations the timing may be critical and own judgement will be required.
• To ensure timely production of medical, clinical or legal documents and following up action points from clinical/medical consultations in line with legislation and established timescales, in many cases timescales will be critical and management of workload and time will be required to ensure deadlines are met thus ensuring fees or complaints are avoided.

Mental Health Act Transfer Coordination:
• Ensure timely assessment and transfer of acutely mentally ill patients under the Mental Health Act 2007.
• Support Consultant Psychiatrists and other colleagues in relation to all patients identified for possible transfer under the Act
• Liaise with Psychiatrists and others carrying out assessments for transfer, ensuring appointments are booked in a timely fashion and carried out to completion
• Continually liaise with mental health inpatient units in relation to those identified for transfer – in particularly with bed managers – to ensure any delay in transfer is prevented or minimised and individuals are transferred as soon as practicable
• Work with prison governors and other managers in relation to this process, ensure the prisons are aware of any individuals likely to be transferred and keeping them involved in every stage of the process whilst respecting patient confidentiality

Clinical Responsibilities
• The post holder will carry a small caseload of complex patients who may require assessment and/or transfer under the Mental Health Act.
• The post-holder will work closely with Liaison and Diversion (L&D) services on the transition of patients.
• The Mental Health Transfer Clinical Lead will organise specialist workload, planning activities around the needs of individuals and adjusting daily diary according to priorities as they arise.
• Provide clinical supervision to staff members within the Health and Wellbeing Team.
• To act as a specialist resource for students from all professional groups enabling them to develop knowledge and skills around mental health transfers.

Strategic Management:
• To develop Surrey wide operational policies and pathways in conjunction with the HWB team, prison, L&D service and NHS Trusts and Commissioners.
• Participate in long term strategic planning for the service.
• To promote research and development and share good practice by reviewing publications, legislation and networking with colleagues that lead to improvement in quality and service outcome.
• To provide clinical expertise and knowledge to inform strategic planning for future needs of the prison population.
• Collect data and information that will inform the development of a strategic plan that responds to the needs of prisoner health needs.
• Be prepared to participate in the Business Continuity Plan in response to a major incident as requested.

Service Development and Improvement:
• To develop polices and evidence-based practice that supports high quality effective transition.
• Undertake and contribute to Quality Improvement projects as required to develop the service

Management and Leadership:
• The Mental Health Transfer Clinical Lead will deputise, as required for the clinical team leader and the head of service for the HWB team.
• Line manage the Mental Health Transfer Coordinator and contribute to professional supervision and appraisal
• Attend regularly and participate actively in Clinical Supervision in line with CNWL Policy.
• To ensure their own continual professional development, keeping up to date with developments in nursing practice and participating in training programmes identified in an agreed Personal Development Plan.
• To contribute to ensuring all staff receive line-management supervision and appraisals on a regular basis, in line with the Trust’s supervision and appraisalpolicy.
• To contribute to ensuring that clinical supervision is provided, complying each of the relevant discipline’s professional regulatory requirements, and thus ensuring that a high standard of professional practice and good inter-disciplinary working isachieved.
• To ensure that all staff are fully aware of and understand their contribution to achieving successful client outcomes and performance targets for theservice.
• To contribute to ensuring that the HWB team develop, implement & evaluate programmes of care using a problem-solving approach that ensures the needs of each individual patient aremet.

MAIN DUTIES AND RESPONSIBILITIES
Clinical
1. To undertake assessments and interventions (for example neuropsychological tests, self report measures, rating scales, direct and indirect observations and semi-structured interviews of clients, family members and others involved in the clients care), as appropriate to level of training, competence and under live or regular supervision from a qualified practitioner Psychologist.
2. To assist in the development of psychologically based frameworks of understanding and care to the benefit of all clients of the service.
3. To formulate and deliver care, training or research plans within settings where there may be a hostile atmosphere, challenging behaviour or high levels of expressed emotion.
4. To conduct literature searches, collate audit and outcome evidence, analyse relevant data and produce reports to inform and enhance care delivered by the service.
5. To assist in the coordination and running of therapeutic groups, training or research projects
6. If required, to compile risk histories to inform care plans, risk and crisis plans.
7. To work with other team members to improve care, training and research practices and the quality of life of clients.

You will also have proven experience of tendering and contract management. We are looking for expert knowledge of NHS Payroll Conditions and Procedures, the NHS Pension Scheme, current statutory legislation, and knowledge of ESR functionality and developments.

South Central Ambulance Service NHS Foundation Trust provides a range of emergency, urgent care and non-emergency healthcare services, along with commercial logistics services.

MAIN DUTIES AND RESPONSIBILITIES:
• To undertake driving duties of relevant type of Ambulance vehicle to the required standard subject to appropriate training, driving vehicles carefully with every consideration being given to the passenger, any equipment, the vehicle itself , other road users and comply with the Highway Code and Trust Policy guidelines and current Legislation .
• The Ambulance Care Assistant will be expected to plan their journeys, taking into account patient appointment timings along with the routes that they should take to ensure an efficient and effective service.
• The Ambulance Care Assistant is required to undertake daily vehicle checks to ensure that the ambulance vehicle is roadworthy these checks shall include for example. Oil, Water, Fuel, Tyre Pressure and Tyre Tread condition, Lights, Warning Devices etc. any defect to be reported using “vehicle defect book” and record on any computerised system as required in accordance with Polices and Procedures.
• To carry out vehicle equipment checks and restock as required at the start of duty in accordance with Trust Policy and Procedures, ensuring that the ambulance vehicle is properly kitted, cleaned (paying particular attention to infection control) and stocked, faults and omissions reported and all appropriate recording procedures completed.
• To operate any communication system supplied/fitted/ operated by the Trust in accordance with its Policies and Procedures.
• To administer “ambulance nursing” to an appropriate skill level in accordance with training received. This will not include the administration of Oxygen or related therapies outside of the skill set for this role.
• To use the “Moving Patient” equipment provided: this will include the use of all types of carrying equipment such as chairs, stretcher, lifting aids etc. This will require the post holder to risk assess the best way to undertake the moving of patients taking into consideration their needs, both medical and cultural and to follow Health and Safety and other appropriate protocols and procedures.
• To manoeuvre patients into ambulance vehicles using either the patient’s wheelchair or ambulance vehicle equipment and safely secure the patient and their equipment within the vehicle for the duration of the journey to and from treatment centres.
• To transport patients using such assistance as may be required to and from treatment centres as required/specified according to pre-determined schedules working to such contractual standards that may be in place. This will include “End of Life” patients.
• To promote and maintain good internal and external communications links with patients, colleagues and associated services and health service colleagues to provide and support a positive patient experience.
• To report any untoward incident immediately in accordance with Trust Policy.

• Full manual UK Driving Licence with no more than 3 points for speeding (any offence relating to drink/drugs on the licence will be rejected)

Knowledge and Experience

Essential criteria
• Previous experience of working in a customer care role
• Previous experience of using online systems and email

Desirable criteria
• Previous experience of working with patients or clients in a care setting
• Previous shift working experience
• Previous NHS experience

Please note: if you are successful for this role and pass both interview and PELDA stages, you must be able to commit to a 3 week foundation course which runs consecutively at the start of your role.
