Based at the Diabetes Centre at the King Edward VII Hospital in Windsor you will be able to work in a variety of settings including delivery of both specialist and community diabetes services.
You will be an enthusiastic and experienced Diabetes Nurse with a warm personality and great people skills. You will need to be adaptable to change and be able to influence new ways of working.
The Team is currently expanding and now has its own full time dedicated Consultant, so this is an exciting time for you to join the team.
You will be willing to undertake relevant training and be able to demonstrate innovative practice.
You will be responsible for providing high quality evidence-based care for people with diabetes, their families, and carers.
We are looking for an experienced Diabetes Nurse as areas of clinical service provision include
• Insulin Initiation and management
• Emerging therapies
• Insulin Pump Therapy
• Flash Glucose Monitoring and CGMS
• Diabetes in Pregnancy
• Care of young adults including transition
• Provision of Diabetes Care within a secure mental health facility
Areas of education provision include Type 1 and Type 2 structured education and Healthcare Professional Education.
An integral part of the role is to focus on promoting a close working relationship between primary and specialist service providers to implement local and national guidelines for diabetes through positive leadership, communication, and expert knowledge of diabetes to support best practice and high standards of diabetes care throughout the Trust.
The post holder will also support.
• GP surgeries and Primary Care Networks in their management of people with diabetes to reduce variation in Primary Care through practice support and education.
• Community Nursing Teams in East Berkshire including advice and guidance, and home visits to review housebound patients on the Community Nursing Caseload who are on insulin therapy.
• Diabetes Education and training for Community Nursing Teams in East Berkshire
