Professional / Clinical Practice 1 Work in accordance with the Nursing and Midwifery Council Code 2 Provide clinical Leadership and act as a role model in the delivery of high quality patient care. 3 Be able to demonstrate specialist knowledge in symptom management of patients with palliative needs. 4 To provide evidence based care that meets the holistic needs of individual patients and their carers/families, including physical, emotional, spiritual, and practical and pre & post bereavement needs. 5 Promote the contribution of nursing with the multi-disciplinary team. 6 Deputise in absence of the senior nurse 7 Ensure that patient risk assessments are undertaken daily 8 Assess the needs of each patient and formulate, implement and evaluate total planned care in accordance with the individual needs of the patient and the procedures of the organisation. 9 To be aware of the changing needs of patients and their carers throughout the illness trajectory and the need for frequent reassessment. 10 Actively participate in personalising the service to the patients and their families, according to the visions and values of the organisation. 11 Have the skills necessary to discuss difficult situations with patients and families. 12 Communicate effectively with members of Lindsey Lodge and wider Palliative Care team adhering to the principles of confidentiality at all times. 13 Assist in initiating new ideas in consultation with other staff. 14 Participate on own appraisal and performance reviews. 15 Report to Senior Nursing and Medical staff the condition of the patients and give and receive instructions, ensuring that they are carried out. 16 Deliver and receive reports and ensure that clear channels of communication are maintained. 17 Be aware of and adhere to all Lindsey Lodge policies and procedures. 18 Maintain accurate clinical records including the use of those on electronic patient records 19 Take responsibility for the general running of the unit, the care of patients and families and the supervision of staff 20 Undertake medicine rounds in line with Lindsey Lodge hospice Management of medicines policy 21 Maintain effective discharge planning. 22 Work in other departments as and when necessary. 23 Contribute to the evaluation of clinical practice through audit. Management and Leadership 1 Be aware of Clinical Governance 2 Contribute to identifying and agreeing care standards, monitor, evaluate and audit the implementation and effectiveness of these standards. 3 Contribute to the implementation and monitoring of Hospice Policies and Procedures, Health and safety Guidelines and professional regulations and guidelines. 4 Ensure that all accidents and incidents are reported to the appropriate authority and relevant Lindsey Lodge policies are followed and act with a duty of candour act all times. 5 Contribute to the maintenance of a safe environment in accordance with Fire, Health and Safety, Security and Manual Handling policies and give guidance to others regarding this. 6 Ensure that economical and effective use is made of all resources. 7 Be familiar with the equipment in the unit and ensure its correct and safe use. 8 Maintain accuracy in record keeping and be aware of the legal importance of those documents. 9 Discuss pertinent issues with team and especially the senior nurse or deputy. 10 Manage a caseload. 11 Take responsibility for leading the clinical team when required. 12 Take responsibility for training and supervision of colleagues who are working on the same shifts. 13 Be involved with induction of clinical staff as appropriate 14 To make professional decisions regarding clinical care and the management of the unit in the absence of the ward manager. 15 Appropriately manage organisational Clinical Change 16 Contribute to Quality improvement across clinical areas Personnel 1 Exercise positive leadership within the team and help to maintain high standards of discipline and care. 2 Report to senior staff incidents on which action and advice is required 3 Maintain good relationships with Lindsey Lodge staff and those from other disciplines ensuring the reputation of Lindsey Lodge is upheld at all times, 4 Help to introduce and support new members of staff. 5 Act as a mentor to more junior nursing staff and Associate Nurses. 6 To attend & actively participate in the regular Clinical supervision programme offered at Lindsey Lodge. 7 To be familiar with, and adhere to, all policies and procedures and ensure that others staff do likewise. 8 Attend relevant unit meetings and if unable proactively seek communication from the events. 9 To be aware of the reputation of Lindsey Lodge and ensure that this is upheld. 10 Ensure that no colleagues, potential employees, patients or carers are harassed or receive less favourable treatment on the grounds of disability, age, sex, sexual orientation, marital status, race, colour, religion or ethnic/national origin. Education 1 Identify own Learning and development needs, based upon nationally and locally agreed requirements for the delivery and development of palliative care. 2 Be actively involved in education 3 Promote a positive clinical learning environment 4 Participate in own appraisal performance annually, keeping to date with nursing research, national guidelines relating to Palliative care. 5 Participate, where appropriate, in helping to develop the skills and knowledge of others. 6 Participate in mentorship, induction and other educational programmes when required. 7 Undertake mandatory training sessions Ensure that specialist knowledge and skills are kept up to date. 8 Act as link nurse in area of special interest - Developing knowledge and cascade training is delivered. Communication 1 Ensure that all communication is effective. 2 Interact on a regular basis with patients, families and staff and members of the primary and secondary care teams. 3 Develop skills in supporting patients and families during the illness and the bereavement period. 4 Participate in multi-disciplinary meetings when required to liaise and maintain excellent communication between the team both internally and externally. 5 Participate in case conferences and Discharge planning when required. 6 Attend and participate in organisational meetings to ensure awareness of developments within the area of practice and organisationally. 7 Promote positive communication and relationships with the public. Other 1. The post holder may be required to rotate onto nights and may be required to rotate to different departments in the hospice, according to the needs of the service.
