An exciting opportunity has become available in the East Integrated Neighbourhood Team (INT) for a senior administrator to join our Camden Integrated Community Healthcare (CICH) workforce in full time capacity. As a Multidisciplinary Meeting (MDM) Coordinator working in a Camden INT, you will play a crucial tole in connecting all members of the Borough-wide multidisciplinary teams, including GP practices and other health and social care partners. We are proactive in looking for innovative people who will facilitate the delivery of excellent integrated working, as well as support the administrative functions for the smooth running of the East Camden Integrated Community Healthcare Team, working closely with the Operational Lead and other administrative and clinical staff.

Following the NHS long term plan, CICH is proud to lead on putting neighbourhoods at the heart of integrated care. The service is set up to deliver health interventions within the framework of the neighbourhood model, working in partnership with other key primary care providers and the local voluntary care sector organisations. This role will be based at East Integrated Neighbourhood team based at Kentish Town Health Centre, however the post holder would be expected to work across our Camden teams according to the needs of the service.

Co-ordinate and facilitate the function of all MDM meetings across the Neighbourhood and cover for colleagues across Camden; which will include the Borough MDM, 3 neighbourhood meetings in the respective neighbourhood of Camden (North, South and West), and be supportive as an integral member of co-ordinated complex care for frail and elderly patients in Camden.

Provide administration support to the Older Person’s MDM members as part of the patient pathways across Camden and liaise with cross border MDM’s i.e. Islington, Haringey, Enfield, Barnet, to ensure all referrals are managed accordingly and in line with any Memorandum of Understanding for Out of Area/ Cross Border Patients may be in place at that time.

Provide administrative support to the East CICH team in collaboration with other administrative staff. Training and support will be provided, as well as regular supervision.

Manage all referrals for review by the MDM. The referrals are received from Health and Social Care professionals.

Inviting the referrer and patient’s GP to relevant MDM meetings where the patient will be discussed.

Ensure that actions are followed-up using action logs.

Ensure that the patient case list is populated and kept up to date.

Email respective care plans and referrals to all MDM members, 24-48 hours ahead of the meeting.

Ensure all referrals are present on EMIS web and that EMIS is kept up-to date.

Providing relevant caseload data and reports.

Cover for colleagues across Camden when needed to co-ordinate and facilitate the function of MDM meetings across the Neighbourhood.

Provide administrative support to the East CICH team in collaboration with other administrative staff.

Although the successful applicant with sit across the whole of Croydon CAMHS, the postholder will share leadership responsibilities within the Croydon CAMHS Treating Team. Work with peers to ensure visible representation of systemic discipline and provide consultation. Support in chairing multidisciplinary meetings, and participate in child protection case conferences and reviews, etc. providing professional advice on individual care programme planning and other clinical matters as necessary. To have overall management responsibility for the delivery of care within an Early Intervention in Psychosis model of service delivery, in accordance with Trust policies and procedures. Brandon exists to enable children, young people and adults with a learning disability, autism, or both, to live life in the way they choose. We provide high-quality, individualised support to around 1,600 people and have a staff team of over 2,000. “First for Mental Health NHS 111 Press 2” (NHS111 MH) is a nationally mandated programme overseen by NHS England. Locally, this is being delivered via the South London Partnership collaboration (SLP – Oxleas, South West London and St Georges and SLaM) with SLaM as the lead provider on behalf of the SLP. The South London model will be co-ordinated through a single hub which will receive calls and triage appropriately. The hub will provide a 24/7, all-age service for people who need advice and support with a mental health need.

The hub will be able to refer directly into secondary care services for those who need an urgent assessment e.g. Local Crisis Resolution & Home Treatment Teams. The numerous benefits of NHS 111MH includes, fewer 999 (immediate suicide risk) calls to the ambulance dispatch queue, a reduction in A&E attendances for those that are seeking help with a mental health need, fewer referrals to primary care and a large proportion of calls now able to be triaged and managed over the telephone, saving time for patients and staff. As well as the new 111 press 2 service we have the opportunity to serve a pilot initiative as part of this hub, this is a joint NHS and Metropolitan Police Service pilot looking at alternatives to the use of Section 136 across London



Clinical
• Answer all calls received in a professional manner, screen and prioritise these calls appropriately.
• Demonstrate good risk assessment and management skills, ensuring that the decisions made regarding calls are in the best interest of service users’ needs and safety. This may include transferring calls to appropriate specialist skill sets, e.g. CAMHS.
• Respond to calls within the time limits set out in the service’s operational policy.
• Undertake clinical triaging of calls to the service deciding on the correct pathway for the call.
• Refer/signpost callers to the appropriate downstream services, including non-statutory, third sector services.
• Maintain a good public image of the Trust and the service by dealing with calls in a professional manner.
• Be aware of appropriate referral and escalation procedures in relation to difficult calls, and utilise appropriate frameworks such as Trusted Assessment.
• Maintain clear and effective liaison and communication with other services.
• To be responsible for the maintenance of accurate and comprehensive patient records.

Managerial

This is a programme which provides an opportunity to engage with young people and families at an earlier stage in their journey with CAMHS. KIT will offer a range of interventions, information and support for those waiting for assessment and treatment and a way to clarify what Croydon CAMHS offer and what service users should be able to expect.
• To provide specialist clinical psychology skill, therapeutic input and consultation to other staff members to support their assessments and treatment planning. Children will have a range of presentations including anxiety, low mood, land may have suspected or diagnosed neurodevelopmental disorders

Croydon CAMHS Treatment team

The post holder will sit in the Croydon CAMHS Treatment Team which is comprised of clinical psychology, systemic therapy, nursing, arts psychotherapy, , child psychiatry and therapeutic social work. Practitioners within the team offer a higher level of clinical input, for children and young people with more complex mental health needs that are referred to the service. Treatment offered by practitioners in the team is evidence based and involves children, young people, their families and professional networks.

The team works very closely with the full range of professionals in the networks for children and young people including education, voluntary agencies, inpatient services and specialist Tier 4 clinics. T

A Vacancy at South London and Maudsley NHS Foundation Trust.


This is a substantive post for a Consultant Psychiatrist in General Adult Psychiatry that has become available due to the move of the long term post holder to another service within the Directorate. The post is full-time (9am-5pm, Monday to Friday). The successful candidate will be part of an active Consultant body and will join the Borough Consultants on call rota.

Croydon Central Locality Team is one of three generic Community Mental Health Teams (CMHTs) in the Borough. Community Mental Health Teams are geographically aligned services; one covering the north of the Borough, one covering the central area of the Borough and one covering the south of the Borough.

The post-holders will provide Consultant clinical leadership to the service and will assume consultant psychiatrist responsibility for their proportion of patients being looked after by the team. This includes assuming Responsible Clinician status for patients under their care, including patients subject to Community Treatment Orders, taking a lead on compliance with the Mental Health Act and CQC requirements. They will participate, directly and indirectly, in the assessment of patients referred to the service and in drawing up care plans which will address the patients’ psychiatric, physical, and social needs. They will carry out detailed psychiatric assessments of patients as clinically indicated to allow accurate diagnoses and contribute to their treatment plans, including pharmacological management of psychotropic medication. The post-holder will forge good working relationships with other psychiatrists across the Directorate in order to encourage appropriate referrals and assure a cooperative approach to the care of patients. They will be linked in with other Consultants and supported to join peer groups. They will also provide consultation and advice to members of the MDT and primary care colleagues.
