The Churchill pharmacy and renal team are looking for an experienced Pharmacy Assistant looking looking to expand their skillset with a move from Community into Hospital Pharmacy, or are looking to progress further in your career within a Hospital setting. Ideally you will have obtained your Specialist BTEC Level 2 Principles and Practices for Pharmacy Support Staff Pharmacy Service Skills NVQ Level 2 (or equivalent GPhC recognised qualification), although this is not essential and support through the qualification process can be provided (successful applicants who do not hold the required qualification will be paid at band 2, rising to band 3 on successful completion of the qualification). Many of our staff progress from this level to become Pharmacy Technicians or other roles within the wider Pharmacy team. This position is a blended role involving both the dispensary, and multidisciplinary working with the renal team on a rotational basis at the Churchill Hospital.

The Dispensary team are supported by ever evolving Technology, with electronic prescribing and a robot to support the labelling and dispensing of medications. The successful applicant will be trained in the full use of the robotic system. Key duties of this role include dispensing medicines according to procedure, using the Pharmacy computer system to produce dispensing labels, preparation of emergency drug boxes, maintaining accurate medication records and assisting in efficient stock control management.

RESPONSIBILITIES:

Principle Duties / Responsibilities
• To provide extensive administrative support to the clinical staff in SDS and RED department.

The pre admission assistant will have an important role supporting the specialist pre admission nurses to be able to complete their assessments. For example, assisting them in preparing the clinic before and assisting with patients during face to face assessments. The nurses also complete many of their assessments via phone. The pre admission assistant will make sure they have all the information they need in advance of these assessments. This presents an exciting opportunity to work with other members of the MDT and communicate with the patients. Requesting and organising test results from a variety of other hospitals, GP surgeries etc. Currently the team comprises of 2 full time pre admission nurses and one team administrator. Overseen by a senior cardiac nurse as team lead.
• To keep the clinic well maintained, stocked and tidy, ready to welcome patients, while adhering to health and safety policies.
• To perform a range of cardiac physiological procedures, and to keep them accessible to the Oxford Radcliffe Hospitals Trust and local GP practices.
• To triage patient’s notes following established criteria and to highlight to the PAC nurses those patients who need a nurse review.
• To complete patient’s notes review under the supervision of the PAC nurses.
• To make telephone calls to confirm face to face appointments with patients to avoid “did not attend” in our clinics.
• To liaise with other staff within the service, Oxford University Hospitals, GP Practices, and other local NHS Trust organisations to ensure that patients receive cardiac physiology procedures requested by clinical staff in the delivery of health care.
• To ensure the collection and availability of administration and clinical data is completed in a timely manner for the smooth running of the clinic.
• To scan on the Electronic Patient Records administration and clinical data received from other NHS Trusts.
• To liaise with the Cardiology PAC clerical staff and cover the PAC answer phone and the PAC email inbox during periods of leave.
• To be a member of the interdisciplinary team, contributing to and delivering effective, quality care of cardiac patients.

· To undergo regular review and completion of the PAC competencies within the first 6 months of starting.

· Without immediate supervision, use appropriate electro-medical equipment (E.G. electrocardiograph, sphygmomanometer, ambulatory recording systems) in wards or departments throughout the organisation to obtain physiological data from patients.

· To check bloods result and flag to nurses deranged values (no need to interpret the values).
• To obtain patient consent for the procedure, accurate anatomical placement of electrodes, labelling correctly.
• Maintain and update as appropriate, knowledge and skills required to record and provide first-line analysis/interpretation of results from a range of physiological procedures, like vital signs, bloods, etc. under supervision.
• Contribute to the maintenance, development and implementation of local operating procedures and policies.
• To ensure Trust protocols are adhered to when labelling and sending samples
• To be effective in prioritising workload e.g. urgent patients added to procedure list.
• To access and collate results from the computer and ensure that they are available for medical and nursing staff whilst ensuring patient confidentiality and disposing of records appropriately in line with data protection protocols.
• To be proactive in ensuring that cardiology preadmission, medical and nursing staff are aware of any abnormal results.
• To keep service lead fully informed and to seek advice and support when indicated.

• To perform, accurately record and report patient observations (as directed by cardiology preadmission nurses) -temp, pulse, Bp, following completion of relevant competencies/training.
• To be able to identify when to carry out and report observations, should a patient’s condition alter/deteriorate
• To perform additional tasks (as directed by pre-admission staff) following completion of relevant competencies/training.
• To demonstrate good communication skills when dealing with patients, relatives and members of the multidisciplinary team utilising sensitivity, empathy and ability to manage conflict.
• To assist with patient moving and handling using correct methods and equipment as required (as per manual handling policy) and completion of appropriate paperwork.
• To be fully conversant and adhere to infection control and COSHH policies to prevent cross infection and hazards.
• To demonstrate skills at time management and prioritisation of workload with support from. service lead
• To participate in surveys/audits as requested by Cardiology preadmission staff or Cardiology Ward staff.
• To demonstrate the ability to concentrate for long periods, whilst maintaining accuracy despite frequent interruptions.
• To undertake mandatory, statutory and local training as required by the Trust and directorate.
• To be flexible/willing to assist all areas within Cardiology preadmission e.g. at times of sickness or increased workload.

To support the department’s role in patient catering by helping to analyse, code and provide allergy information for patient menus; under the guidance of a dietitian, help patients requiring a special diet to choose and obtain their required meals / snacks / etc; act as an intermediary between the dietetic and catering departments and enhancing communication to achieve the optimal provision of nutrition for patients in hospital.

To assist Dietitians and Secretaries in completing general administrative tasks, such as registering or amending patient registrations on the Abbott Hospital to Home Service, keeping track of feeding pumps given to patients, stock control, data input or letters.

To assist Dietitians to enable patients to be discharged from hospital promptly, for example to provide patient information, feeding pump or equipment, needed by patients prior to hospital discharge; and to co-ordinate supplies of nutritional supplements or tube feeds for patients.

After training, to support dietitians in delivering aspects of dietetic care by reviewing, monitoring, and advising patients and undertaking related administration.

To participate in project work and be involved with education and training of staff, such as housekeepers in aspects of nutrition or nutritional screening e.g. MUST training.

To undertake and oversee the ordering, delivery and payment of nutritional products (e.g. tube feeds and sip feeds).

CLINICAL

1) After training, to review and monitor a small case load of named patients under the supervision of a dietitian, e.g. patients on established tube feeds, limited nutritional intake and those following therapeutic diets. This may involve:

a) Gathering relevant clinical information from electronic patient records, nursing staff or housekeepers, patients and carers

b) Liaise with nursing staff and other members of the multidisciplinary team, for example, Speech and Language Therapists (SALT), Occupational Therapists about the patient’s nutritional plan

c) Calculate patients’ intake from food record charts, which may involve using a dietary analysis computer package

d) Monitor patient in order to achieve optimum nutritional intake

e) Assess patient’s tolerance to tube feeding

f) Regularly liaise with the supervising dietitian about all aspects of patients nutritional care

g) Record all interventions and actions on electronic patient records (EPR) ensuring that all entries meet the professional standards of the British Dietetic Association

2) Oversee the completion of daily menus for all patients on therapeutic diets. This involves face-to-face discussion with patients and/or their relatives or carers, working closely with Catering staff.

3) To be involved with education and training of staff, such as housekeepers in aspects of nutrition or nutritional screening e.g. MUST training.

PROFESSIONAL

Participate in the induction of new ward and dietetic staff; demonstrating the role of the Dietetic Assistant as well as informing them about the provision of therapeutic meals, nutritional supplement products and raising nutritional awareness in their ward areas.

If required, participate in training the Diet Chefs, Catering Assistants or ward staff, about the importance of nutrition and educate on the special and therapeutic diets using a variety of teaching methods. It may be necessary to adapt the teaching methods used to facilitate those with limited English.

To be involved in project work including review audits, working alongside dietitians, other health professionals and catering staff, for example, mealtime observation audits, patient mealtime satisfaction survey.

To work alongside Dietetic colleagues, Catering and administrative staff, learning by experience and improving working practice on an on-going basis.

If required, to deliver training to staff and/or patients under the supervision of a Dietitian.

Participate in annual appraisal, identifying own developmental and training needs and setting a personal developmental plan.

CATERING

Liaise with dietitians, patients, the catering department and ward staff to ensure the provision of therapeutic diets, appropriate snacks, supplements and enteral feeds runs smoothly.

Work as an active and contributing member of the dietetic catering group, and provide regular feedback to the rest of the dietetic department.

When required, code menus to reflect changes in food supplied, which will involve communicating with the catering suppliers, catering managers, dietitians and chefs across all four hospitals.

Work with the dietitian to alter menus as necessary, update the special diet manuals and nutrition policy. Disseminate this information throughout the Trust and on the Intranet site.

ADMINISTRATION

Be responsible for the ordering and supply of nutritional products to wards and outpatients. This involves:

a) Conducting weekly stock takes and placing orders for tube feeds and nutritional supplements, with different nutritional suppliers, liaising with customer services and nutritional representatives as necessary.

b) Ensuring all invoices are processed and sent to finance for payment.

c) Maintaining record charts for regular stock checking and billing hospital wards for nutritional supplements and enteral feeds.

Be responsible for maintaining a supply of feeding pumps for use on the ward or at home. This involves:

a) Liaising with nutritional suppliers’ representatives and customer services when requesting a delivery.

b) Sending new pumps and those for repair or servicing to Clinical Engineering.

c) Regularly checking wards to ensure pumps are present and operating efficiently.

Complete appropriate paperwork when patients on enteral feeds are discharged from hospital.

To assist the Department Secretaries in completing general administrative / secretarial tasks. This may include answering and dealing with telephone calls, entering Department patient activity data (‘stats’) onto the department spreadsheet, general department wide administration, etc.

ORGANISATIONAL

To determine priorities for own work area based on clinical need and balance patient caseload with other professional demands, for example.

To collect data as required by the department, trust and outside agencies.

To maintain accurate and up to date documentation of patient interventions on electronic records.

To undertake and oversee the ordering, delivery and payment of nutritional products (e.g. tube feeds and sip feeds).
