You will be responsible for assisting the professional team in providing care for patients in an inpatient environment. The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population’s health and wellbeing and reducing health inequalities. The successful candidate will be a member of the multidisciplinary team, working in a fast pace community urgent care pathway offering a community response to those experiencing a mental health crisis.

As an senior mental health practitioner, you will work within a fast paced urgent community care pathway offering an urgent and crisis response to those experiencing mental distress within the Community.

You will conduct initial CPA assessments, agree and develop collaborative care plans underpinned by NICE guidance delivering a range of interventions based upon individual needs.
