To undertake clinical training and practice development within identified care homes. To work in collaboration with Care home managers to deliver training to care home staff including Sepsis identification, EOL and the use of Whzan, To provide professional support and advice to staff in the Care home setting, and to facilitate education and training to registered care homes using evidenced based care. To plan, implement and evaluate Education needs in care homes involving District Nurses, EPUT care home team members and care home staff To develop education packages using research and current clinical evidence with particular emphasis to Sepsis.
