The Intensive Support Team is an all age, multi-disciplinary service that provides an enhanced model of support to the wider community learning disability services across Lancashire and South Cumbria. The team is a primary focus on supporting people to avoid unnecessary hospital admissions, and to live well within their local communities. The team provides positive behavioural support and dynamic risk support within defined pathways, to avoid placement breakdowns.
The post holder will need experience of supporting people who display challenging behaviour, and supporting risk mitigation and collaborative working with carers, families and other stakeholders. A good understanding of transforming care policy is essential as are adaptability, good communication skills and self-motivation. An experienced clinician with proven skills in working with people who have complex health needs and challenging behaviour and/or Autism, demonstrating practice is evidence based and innovative.
The post holder will be responsible for a team caseload and supervision of other practitioners (registered and unregistered), to provide strong clinical leadership and supervision within the intensive support team.
The post holder will work flexibly between the hours of 8am and 8pm by working a rotational shift pattern including weekend working to meet the needs of the team and the wider network
The Intensive Support team helps people with learning disabilities to avoid placement breakdown and unnecessary hospital admissions, by providing a range of clinical interventions that include positive behavioural support and dynamic risk mitigation.
The multi-disciplinary team includes Nursing, psychology, Social worker, Speech & language Therapy and Occupational Therapy, all dedicated to supporting a team caseload of people needing intensive support.
You will be expected to have experience in supporting people who present with challenging behaviour, as well as experience of community learning disability practice.
