This Registered Nurse - Patient Aligned Care Team provides direct Veteran care and care coordination within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, education, coaching on wellness, disease prevention, and chronic care management. This nurse collaborates with patients to assess and identify needs, issues, care goals, and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery, and health maintenance/wellness.

The Registered Nurse - PACT demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Provides peers with informal constructive feedback for improvement. Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team. Partners with with others to effect change and produce optimal outcomes. This nurse supports colleagues through knowledge sharing to provide safe, quality outpatient care.. • Provides local and multi-state expertise during international trade negotiating, and interprets negotiated work plans and implements work plans at both a local and multi-state level.
• Serves as a PPQ technical specialist, interpreting foreign quarantine regulations, negotiating to resolve international phytosanitary procedural disagreements, and managing export work plans.

MAJOR DUTIES:

• Coordinate initial emergency response, evaluating incidents, soliciting all necessary information, and applying judgment according to interagency guides, protocols and pre-plans.
• Receive and evaluate emergency calls, soliciting information to initiate an appropriate response of emergency services personnel, acquiring critical information (law enforcement, fire, medical search and rescue, etc.) from the public and other public safety agencies.
• Use a variety of specialized computer programs such as NCIC (National Criminal Information Center), NLETS (National Law Enforcement Telecommunications System), and CAD (Computer Aided Dispatch) in the performance of dispatch duties.

The Advanced Medical Support Assistant (AMSA) interact with both internal and external customers providing exceptional customer service and ensuring overall customer satisfaction. The Chief Nurse executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines, influence organizational mission and health care. The Nurse IV is responsible for administrative functions of their assigned nursing units/programs. Functions as an administrator, educator, and consultant utilizing management theory in collaboration with VISN, Chief of Staff, Patient Care Services and other services. Must establish and maintain effective relationships with all levels of medical center personnel and relate effectively with customers, families, and the community as well as program and service leaders at the local and network levels.
