The Elderly Care team with 14 funded Consultant Geriatrician posts (13 in place) currently provide a wide range of hospital and community based services including Acute Frailty services as well as running specialist clinics for patients with Parkinsons disease, falls and general medical conditions. We have recently started a Rapid Access Clinic for Elderly as part of Frailty- Same day Emergency Care (F-SDEC). Inpatient elderly care wards include 5 acute elderly care wards and an established purpose built Acute Frailty Unit with10 Assessment beds, 22 short stay beds and 2 clinic rooms. In addition to existing services, we are seeking colleagues to take on the challenge of developing new models of care for frail elderly people in the community such as Frailty. Pioneering work has already established Integrated Locality Review meetings (MDT meetings, 4/week). Consultants are working closely with existing community teams to proactively assess and manage frail individuals in the community. We are about to open a 22 bedded Acute Unit based in a Community Hospital in Sheppey. The post holder will join the existing team of full time Consultants. The post holder will need to take responsibility for the management of patients admitted to his/her specialty Elderly Care ward (12-15 patients) routinely and those admitted under their care whilst on call for Acute Frailty. The latter would average 12-15 per on call. These patients are then streamed to Frailty Short Stay Unit if early discharge planned (<48 hours) or one of the 5 Frailty Base wards for longer admissions. Acute Frailty Unit has a dedicated Elderly care Team with 2 Consultant Geriatricians supported by their own team of junior doctors and a Multidisciplinary Team. There are 10 funded posts for Middle Grades in the department with currently 6 in post. Elderly care Consultants participate in an Acute Frailty On call service. The post-holder will be paired with another Consultant so that in a 12 week block they provide 2 weeks of weekday on call and 2 weekends on call. Full registration with the GMC and the ability to make decisions at a senior level, the role will allow you the opportunity to continue your personal development, and participate in the teaching, research and governance programs within the Department. All subspecialist interests are welcome. There is an opportunity, in negotiation with the other consultants, for participation in the running of the established clinics such as Falls and Movement Disorder Clinic. The successful applicant will also have an outpatient workload comprising of new and follow referrals for managing complex co-morbidities. Typical clinic would have 3-4 new patients and a similar number of follow-up patients. The Mental Health Nurse (RMN/RNLD) is a key member of the multidisciplinary care team and will be responsible for assessing, planning, delivering and evaluating care, communicating findings, influencing change and promoting health and best practices, patient safety and experience.

The post holder will be professionally accountable to the Executive Director of Nursing, and operationally accountable to the Matron / Clinical Service Manager.
• Act as, or train to become, a Practice Assessor. Support student nurses' learning and development through assessment and feedback.
• Comply with NMC registration, revalidation, and Code of Conduct standards.
• Support the implementation of nursing strategies and adhere to KMPT policies and values.
• Identify, report, and respond to safeguarding concerns.
• Prioritize patient safety through safe practice, risk management, and incident reporting.
• Provide holistic care, including education, medication management, and support for patients and their carers.
• Supervise staff, manage workloads, and undertake specific clinical responsibilities as required.

What you'll need to succeed

• Preferred Experience: Minimum of 2 years of experience in a care setting supporting adults with learning disabilities, and at least one year within the UK. Your chances of success and placement with our clients will increase based on your background experience.
• Ideal Qualifications: Qualifications such as an NVQ L3/2 in Health and Social Care or relevant qualifications in supporting adults with learning disabilities are ideal. (Note: Some homes may require or prioritise candidates with specific qualifications, such as NVQ LVL 3. Mandatory training evidence will be requested, and any required modules will be assigned through our online training platform.)
• A manual driving licence, valid in the UK. You will also be required to drive service vehicles from time-to-time on-site to transport residents (not mandatory but will be prioritised over non-drivers)
• Flexibility: We're seeking workers who are as flexible as possible. We understand your individual needs, such as work, education, and family commitments, and will strive to accommodate them. (Note: Flexibility will be prioritised.)
• Resilience: This role can be challenging, so a strong will, perseverance, and patience are essential when dealing with difficult situations. You must be engaged with the residents in activities, including life sessions, shopping, cooking.
• Effective Communication: Good written and verbal communication skills are necessary. You'll be responsible for writing in logbooks and potentially preparing reports or editing care plans.

The Role
You will support the team to provide essential learning and accessibility support to young people and adults with SEN which includes facilitating their access to and achievement of learning programmes, supporting their access to all College services and College led activities and facilitating personal development of learning/study skills and educational outcomes including those relevant to individual Educational Health & Care Plans (EHCPs)
You will be required to provide person-centred learning support to students with severe and complex learning difficulties and/or disabilities which may include supporting them with personal care where necessary. You may work with individuals or small groups and encourage students to engage with activities or work independently.
Skills and Experience
You will have good literacy and numeracy skills and have the ability to establish and maintain good working relationships with students, teachers, parent/carers and, other agencies involved with the students. Experience of working in a multi-cultural environment and an understanding of safeguarding and discrimination issues and the difficulties faced by those with learning difficulties are essential.
You must be willing to assist learners with regards to their personal care and mobility, taking instructions from learners regarding their support needs. You will have excellent interpersonal and communication skills with the ability establish good working relationships. You’ll also have exceptional organisational skills combined with good IT knowledge and sound administration skills. Experience of working with SEN learners in a secondary or FE environment is desirable.
