The functions of the RN Care Coordinator/Maternity Coordinator include, but are not limited to
the following:
- Practices in accordance with Veterans Health Administration (VHA) Handbooks & Directives
that provide guidance for women Veterans' health care (1330.01 & 1330.03). - Implements strategies to increase compliance with VHA health care guidelines and
performance measures for women Veterans. - Coordinates and tracks gender-specific care/maternity care, of patients throughout all aspects
of the STVHCS medical center, including Community Based Outpatient Clinics, as well as
community and/or military partner providers and medical centers. - Demonstrates effective coordination of care of women Veterans requiring maternity care and
regular interval assessment of care. - Assists with initial VA consult referrals, assists with VA or community services for the patient
with an appropriate provider, to include coordinating any subsequent appointments or follow-up,
as needed. - Maintains database to monitor the delivery of maternity services and coordination of care and
tracks maternal and fetal outcomes. - Educates patients on a variety of gender-specific topics, to include maternity specific topics, as
well as authorized VA and community-based resources for gender specific care and billing
processes. - Consults with and collaborates with interdisciplinary services to meet the needs of women
Veterans. - Contacts pregnant veterans on a regular reoccurring basis throughout the duration of
pregnancy and the postpartum period to assess clinical and/or social care needs, facilitating
care and services between VA and non-VA care providers. - Assists veterans who have presented to VA or non-VA emergency department for gender specific
maternity related emergencies, to secure appropriate follow-up care such as lab,
ultrasound, emotional support, and/or other care as needed. - Ensures seamless and timely return to Primary Care following delivery by assisting with a
follow-up appointment within designated timeframe. - Documents patient care interactions accurately, succinctly, and timely.
- Assists with managing Teratogenic medication communication, prescriber education and
electronic medical record alerts and counseling documentation. - Completes initial and re-assessments of patient condition and response to treatment and other
clinical findings to include mental health screenings, and other screenings. Initiates follow-up as
needed. - Provides a full range of nursing care to patients with complex care needs.
- Applies problem-solving skills and communication skills; works as a member of the Women
Veteran Program, Comprehensive Primary Care-Women's Health Team, educating staff,
learning from staff and improving processes resulting in improved patient care. - Participates in committees to ensure an environment of dignity, respect, privacy and safety for
women Veterans.
