POSITION SUMMARY:

• Works in the home and/or community with assigned client(s), as designated by the Program Coordinator and/or the Regional Director.
• Participates in Individual Support Plan meetings when possible and in RN visits or service monitoring visits.

Position Summary: 

The Nurse Practitioner (NP) will function as an independent healthcare provider working collaboratively with a multidisciplinary health care team to provide comprehensive Palliative Care both inpatient and outpatient (office visits, telehealth, and home/ALF/SNF visits) while demonstrating a high degree of clinical expertise. Experience or advanced training in Palliative Care or related specialties is preferred. The Palliative Care inpatient service is staffed 7 days per week and the provider would be expected share in the weekend/holiday rotation with the other Advance Practice Clinicians (APCs). The Palliative Care outpatient service is available on weekdays and is currently purely consultative with a goal to add future prescribing capabilities.

The provider would be part of a multidisciplinary team consisting of a Physician Medical Director, Advance Practice Clinicians (APCs), and Social Workers to consult regarding symptom management related to life-limiting illnesses (eg. pain, dyspnea, nausea, constipation, psychosocial stress, etc), end of life care, goals of care discussions and serious illness planning/communication, advance care planning completion, and hospice transitions. The provider would be responsible for providing direct individualized patient care and treatment with responsibility for assessing, examining, diagnosing, and analyzing the patient's current health condition and developing short and long-term goals and treatment plans. The provider functions within the defined scope of clinical practice as outlined by applicable state law, licensing, certification, regulations, institutional policy, and practice agreements.

Title of Position: Plano Event & Food Service Support

Summary of Position:

The Plano Event & Food Service Support position partners with Campus Ministries and Support Teams to help organize, coordinate, and execute campus events & food service needs.

The ideal applicant will be skilled in handling a wide range of administrative and logistical tasks. They will also be highly organized and good at multi-tasking. Experience in planning, coordinating, and executing events is required. Exceptional communication, interpersonal, time management skills, and effective execution are a must.

Principal Job Responsibilities:

• Ensures the success of each event by championing policies, procedures, guidelines, and codes pertaining to event planning & food service support
• Builds and maintains effective and positive relationships with all stakeholders
• Provides oversight and direction for all on-site teams including outside vendors while also providing the necessary tools required for them to perform tasks efficiently and effectively
• Manage event planning by possessing a complete knowledge of the Plano Campus and grounds including room capacities, room usage, preferable table and chair set ups, room adjacencies, food service allowances, multimedia equipment availability, traffic flow patterns, facility entrances, parking areas and fire code requirements
• Proactively manage event planning software and ensure all event details are accurately represented in the system
• Contract vendors for events and coordinate on site set ups and teardowns
• Maintain cleanliness and organization of kitchen, pantry, and storage areas
• Track, purchase, and restock all inventory related to event and food services 
• Correctly document and submit departmental credit card purchases for event and food service support needs
• Stay within departmental approved budgets when planning for events & food service support needs
• Effectively communicate pertinent information regarding events to necessary departments and ministrie
• Attend departmental, Campus, Central Events, and Café Food Service meetings and trainings
• Be available to assist and serve Central Events at Gateway Conference and/or other conferences as scheduled
• Any additional tasks needed for food and event team support
