Opportunity available for a Advanced Nurse/ Clinical Practitioner to work within an Urgent Care Centre, Across the West Midlands

The team sits within an Urgent Care Centre with the role predominately be responsible for the delivery of evidence-based nursing services, working as part of the service multi-disciplinary team, delivering care within their scope of practice to the entitled patient population. You will be working autonomously, the Nurse Prescriber will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure. As part of this role you will develop, implement and embed health promotion and wellbeing programmes and develop, implement and evaluate individual treatment plans for acute and non-acute conditions. You will support the clinical team in the development of individual treatment plans for chronic disease patients and Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health. Providing routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF is a must and you will be able to prescribe medication in accordance with PGD and within own scope of practice and review medication for effectiveness, need and national guidance

We are looking to recruit a site services officer to support our site teams across the trust.This role plays a crucial part in the smooth running of our schools. Under the day-to-day supervision of the Senior SSO and under the direction of the MAT Facilities Manager and schools SLT you will support the site team in the provision of all site service activities during the week and the weekdays as cover.
This includes janitorial, cleaning, maintenance, and monitoring duties and ensuring the site is safe, well-maintained, attractive, and a suitable environment for the educational and community activities taking place on-site and across other schools in the MAT as required.

Working hours: 08.20 – 15.20

Additional hours may be possible after school or during lunch break

We are seeking to appoint an experienced, kind and dynamic Learning Support Assistant who will support in EYFS and Key Stage 1, in line with the ethos of the school.

The post is based in our rural village school in Clapham and Patching, which is in the heart of the South Downs. We pride ourselves on our ethos, as we aim to provide the best possible education for all our children. This is a great opportunity to work as part of a dedicated and professional team supporting children at this small, friendly village school.
