This includes but is not limited to Therapy (SDHC @home team and PCNs, Care (Reablement and Brokerage), Bed-based rehabilitation, short and long-term placement. Home First Clinician of the Day is responsible for triaging, leading on complex discharges, supporting the wider team, ensuring all systems are updated and ongoing service developments keeping the patient at the centre.
