As part of your role you will directly support service user with care and activities.
Team Leader Duties:
- You will lead the onsite care team within our Supported Living Services to provide person centred care and support to older people at the service. This includes providing respectful personal care, emotional support and practical support with activities of daily living,
- You will supervise staff and manage the service rota to deploy staff time efficiently in line with service user needs and preferences,
- You will liaise with families and professionals to plan and review care to maximise outcomes, ensuring the individual is always at the centre of their care and support.
We are looking for team leader to work alongside our existing management team, based predominantly in Poplar Tree Court. Poplar Tree is a service for up to 18 adults. They adults have mixed needs ranging from personal care, moving and handling and also residents who live very independently.
The team leader will be required to lead a team of support workers to ensure that the service provides outstanding care to our residents. They will be responsive to issues, take ownership of audit and quality assurance work, health and safety, and line manage a number of support staff. They will work alongside the registered manager to lead on all aspects of CQC Regulation and internal governance. They will ensure good communication with the wider team and ensure our service users enjoy a varied range of person centred activities, whilst living in a high quality home.
The right candidate will be person centered, organised, and have experience of line management as well as working within a CQC registered service supporting adults with a range of health needs.
