Lancashire & South Cumbria Trust Pennine locality has an exciting opportunity for a Team Co-Ordinator to join our community rehabilitation team. Applicants should have a relevant professional qualification (eg. BSc in Occupational Therapy), operational experience and clinical experience working with older people at risk of falls.

You will join our teams in supporting patients in a community setting and must be highly motivated leader, a conscientious team player with excellent communication and analytical skills.

You will have at least two years’ experience working in a community setting at band 6 level with excellent rehabilitation and falls management knowledge.

Supported by senior leaders, we are looking for enthusiastic, compassionate, motivated and creative individuals who can join our vibrant team. An ability to work independently, be a positive role model and demonstrate the Trust’s values is essential.

You should have the ability to travel within the Blackburn with Darwen area and be able to manage and prioritise your own workload as well as monitoring the caseloads of the teams, maintaining waiting lists and ensuring all patients are seen within a timely fashion ensuring clinical need and targets are met.

You will have responsibility for staff supervision, including PDR and clinical supervision. You will also need to be able to have an overview of quality indicators for services.

The successful applicant will be expected to be flexible to meet the needs of the service. A commitment to working within a multidisciplinary environment with close liaison and collaboration with other professionals and carers will be vital. You will be offered supervision and support by LSCFT professional leads, as appropriate.

Work is ongoing to transform adult physical health services across the Pennine Lancashire Footprint supported by commissioners within the Lancashire and South Cumbria system. This transformation aims to ensure service users in Blackburn with Darwen and East Lancashire receive equitable service provision from one service provider. From around Summer 2024, the service provider will be East Lancashire Hospitals Trust. Staff employed by LSCFT will therefore transfer from LSCFT to East Lancashire Hospitals Trust as part of this wider transformation plan.

Appointment is by interview and successful applicants will need to undergo the usual NHS pre-employment checks.
