We are looking to appoint a Consultant in Critical Care Medicine to join our dynamic team at the North Middlesex Hospital. You will get to work in modern purpose built facility with modern and spacious operating theatres housing state of the art anaesthetic delivery and monitoring equipment.

Main duties of the job

The post holder will work with colleagues and the Clinical Audit and Effectiveness department to ensure that relevant clinical audits are carried out.

Consultants have continuing responsibility for the care of patients in their charge and for the proper functioning of their department. They are expected to undertake administrative duties associated with the care of their patients and the running of their clinical departments.

The appointee will be expected to take responsibility for maintaining their continuing medical education to the standard set by the relevant Royal College.

This appointee is expected to contribute actively to developing clinical governance in both departments. The GMC’s Good Medical Practice guidelines form the basis of the responsibilities and standards expected of the Trust’s consultants.

All newly appointed consultants will be issued with an Induction Programme and mandatory training, all aspects of which are mandatory. There is a well-established annual appraisal process for all consultants and non-consultant career grades in the Trust.

It is a condition of employment that all staff have a responsibility for Infection Control. The appointee will therefore be expected to attend mandatory Infection Control courses on appointment and to participate actively in measures designed to reduce Health Care Associated Infections (HCAIs).

The Critical Care Department in the hospital includes an Intensive Care (ICU) and a High Dependency (HDU). ICU has a total of 8 individual rooms with single beds and 2 large rooms with 2 beds each. All rooms in ICU are negative pressure rooms. HDU has 09 beds including 3 single rooms and two large 3 bedded areas. All beds in the Critical Care Complex are equipped with full monitoring facilities and a central monitoring station on each side.
The Critical Care Complex admits close to 1000 patients per year. Median length of stay for the unit is 3.4(1.8-6.9) days. Bed occupancy is more than 90% for majority of time. Turnover of patients is really high hence the unit is a very busy. Of the all the referrals over 90% are from medical specialities. There is a weekly radiology meeting on Thursday morning. Microbiology involved multi-disciplinary meeting is held every day afternoon in the critical care complex. Daily there are two consultant lead ward rounds plus a night telephone ward round. There are two handovers every day directly supervised by critical care consultants. Critical care also provides 24/7 outreach services to the hospital. It is led by a lead nurse for outreach. Critical care provides a follow up clinic bimonthly for identified patients. There are plans in place to increase the frequency of this clinic to once a month.

Duties of the post We are looking for a candidate who can integrate well into the Obstetrics and Gynaecology department. The candidate will be expected to run maternal medicine, antenatal diabetes and general antenatal clinics and have a strong interest in Obstetrics and postnatal care. It is expected that the candidate will operate as part of the existing team, attend MDT meetings and contribute to the management of complex obstetric patients as well as provide leadership for postnatal care. Currently there is a maternal medicine service run by consultant obstetricians, Mr Harry Gibson and Miss Charlotte Depoers, with Dr Yasmin Jamil, consultant obstetric physician and Dr Zara Sayar, consultant haematologist. There are well-established links with other hospital specialists as required, and clear pathways to tertiary specialist care through the North Central London Maternal Medicine Network. The service is delivered via a weekly maternal medicine clinic (haematology 1x/month, obstetric physician 3x/month, approximately 180 new patients/year) and a weekly diabetes clinic (400-450 new patients/year), in addition to inpatient referrals. The team is able to offer pregnancy care for category A and B conditions as well as tertiary level care in category C haemoglobinopathy conditions. There is a weekly MDT meeting of neonatologists, anaesthetists, midwives and obstetricians where complex cases are discussed, and a monthly MDT with the regional network. The specialist pregnancy diabetic service includes a weekly multidisciplinary antenatal clinic staffed by Dr Michaela Rossi, consultant endocrinologist, Miss Charlotte Depoers, Mr Harry Gibson and Miss Elly Tsoi, consultant obstetricians. Pre-pregnancy counselling is provided for women with pre-existing diabetes or other medical conditions (such as sickle cell disease) which could be adversely impact the mothers health during pregnancy. Barts Heart Centre has 10 operating theatres, 10 catheter labs and over 250 cardiac, vascular and thoracic beds, creating one of Europe’s largest Cardiothoracic Centres.

The new hospital contains 58 critical care beds. 2 ICUs and 1 HDU comprising 42 beds form the surgical cardiothoracic surgical critical care unit, adjacent to theatres.

A fourth ICU of 16 beds on the 6th floor provides dedicated General ICU, admitting complex cardiac patients, together with cardiology and respiratory patients in addition to patients suffering from haematological disease.

Following changes to the North East London School of Anaesthesia training programme, we are now able to offer a few short term fellowships in cardiothoracic anaesthesia and critical care.

These posts would be suitable for those individuals aiming to complete the CESR in anaesthesia or who simply want to increase their experience in cardiothoracic anaesthesia. Successful candidates will be required to have undergone extensive basic training in anaesthesia (equivalent to core anaesthetic training in the UK) and ideally have some experience at specialist registrar level. They would be expected to have the Primary FRCA or equivalent as a minimum, but would not be expected to have undertaken any cardiothoracic training prior to joining the department.

The workload would be split between time spent in the cardiothoracic critical care units and operating theatres and cath labs and would involve all aspects of perioperative care of the cardiothoracic surgical and cardiology patient.

Locum consultant position in Paediatric Intensive Care

The Department of Paediatric Intensive Care is seeking one locum consultant full-time equivalent appointment. This is to cover consultant retirement while the consultant workforce for paediatric critical care is reviewed. Applicants are sought from a background of anaesthesia or paediatrics with appropriate super-specialty training or experience in paediatric intensive care, with a proposed start date in November 2024.

The Evelina PICU is a large unit admitting approximately 1100 children per year into a 20 bedded tertiary, and 5 to 10 bedded level two critical care area- all situated over 2 floors. The critical care unit has the capacity to flex up to 33 beds during the busiest times.
Forty-five percent of the case load relates to cardiology and cardiac surgery, including provision of post-cardiotomy and respiratory Extra Corporeal Membrane Oxygenation (ECMO). Fifty percent of admissions are as acute emergencies from the South Thames Paediatric Network catchment covering the South East of England. The South Thames Retrieval Service (STRS) is integrated with, and delivered from within the staffing structure of the PICU. Clinical management of referrals is led by the on-call PICU consultant team.
The PICU and retrieval service, STRS, act as the critical care lead centre and hub of an extensive managed clinical network of district general hospitals (DGH) and other paediatric intensive care units in South London. This critical care network manages critically ill children in South London and the South East of England under the banner of the South Thames Paediatric Network (STPN).

This is an excellent opportunity to join a team based in a central London location in a large quaternary Paediatric referral centre and closely associated with partner paediatric intensive care unit at the Royal Brompton site, working as one paediatric critical care directorate, and co-located neonatal and adult intensive care services at the Evelina and St Thomas’s hospital, sharing the same site in central London. All newly appointed PICU consultants across both Evelina and RBH sites will be expected to be able and willing to undertake clinical service on either site.
