RESPONSIBILITIES
- Flexible work schedules
- 5 days' work week, Monday to Friday
- Working hours: 9 am to 6 pm
- Able to travel around Singapore
- Salary paid daily-rated
- Subject to CPF contribution
- Opportunity to convert to full time position
The Senior Care Associate (SCA) provides Home Based Therapy and Intervention and assistance to clients in their daily living and performs tasks according to the care plans developed by social service and/or healthcare professionals under Community Based Support Services (CSS).
CSS is the dynamic and nimble service delivery arm in MINDS that response to emerging needs of PWIDs and Caregivers in the community. Examples of services include Me Too! Club, Homebased Care Services, Caregivers Support Services, Special Student Care Centres and Appropriate Adult Services to enhance integration and community living of caregivers and PWIDs in the community.
With the support of a team of Allied Health Professionals, the SCA maintains up-to-date documentation of clients' conditions and provides inputs for the development and review of care plans. He/She conducts activities for clients to promote independence, health, wellness, and quality of life and provides guidance to clients on personal and social skills, self-care and community living skills.
A caring, patient and responsible professional, the Senior Care Associates works in various voluntary welfare organisations, communities and institutional settings.
Provide Client Training and Support
- Provide personal care assistance to clients in enhancing their daily and community living skills.
- Support Allied Health Professionals (AHP) in providing assistance to clients in managing their rehabilitative needs.
- Perform activities according to care/training plans.
- Provide inputs for development and review of care plans
- Maintain up-to-date documentation of clients' conditions and progress
Maintain order, safety and cleanliness of the care environment
- Respond to unusual or challenging behaviours of clients
- Recommend measures to mitigate risk to clients with support of (AHP)
- Follow advisories with regards to basic health, hygiene, and safety to provide a safe and healthy physical environment.
- Ensure Training Materials is maintained in a clean and working condition.
- Adopt proper documentation of incident reports
- Apply technology effectively for work and teaching
Conduct programmes and communicate effectively with caregivers.
- Conduct social, recreational and educational activities to promote independence, health, wellness, and quality of life
- Communicate with caregivers and work collaboratively on training plans
- Identify areas of improvement in programmes, activities and trainings
QUALIFICATIONS
- Minimum N Levels/ITE
- Added advantage for those with experience in education/training of persons with intellectual disability / or multi-disciplinary team environment.
- Self-motivated and fully able to participate in on-site and off-site activities
