Applicants should be HCPC registered dietitians, with relevant post registration dietetic experience. The post holder should ideally have experience of working within a weight management service.

Are you looking for a rotational post in the community in Hull and the East Riding? We are able to offer a rotational post that currently covers Hull and East Riding Community Rehabilitation and Hull and East Riding Intermediate Care. Our Community Rehabilitation team work with patients who have a variety of physical and neurological conditions, with the overall aim of them being able to maintain or improve their functional ability after illness/injury or a deterioration in their condition.

We work alongside Physiotherapists and Associate Practitioners to achieve patient goals, which can be as varied as being able to prepare a meal independently following a brain injury, to being able to get up the stairs to use the bathroom following a hip fracture.

The aim of our Intermediate Care service is to prevent hospital admissions and to facilitate discharges. We have several bedded units in the community where we work on rehab goals with patients before they go home. We also carry out rehab with people in their own homes, and we respond to crisis when people are struggling at home .

We work alongside Physios, Nurses, Pharmacists, GPs and Associate Practitioners in our MDTs. The working pattern in Intermediate Care includes working some weekends/public holidays and evenings until 8pm.

We have recently begun a cross organisational rotation with our Humber Teaching NHS Foundation Trust colleagues, therefore are very pleased to be able to support staff who are wanting to have experience in both physical and mental health. We are very happy to support and develop new staff, including those who have recently qualified and are developing their professional skills. We have a supportive Band 5 peer group who meet regularly, and an OT Professional Development group also.

Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes.

The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England’s website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS. The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull.

In this role you will be working in providing urgent medical care to frail patients acutely unwell in their usual residence, including care homes. This is an amazing varied opportunity for an independent prescriber with advanced practice qualifications who is passionate about caring for older and frail patients, who also has the drive to help further develop a high profile service care within the community.

In this role you will work as an expert autonomous practitioner supported by the JB-CFS; assessing and delivering the care and treatment of older people and those with frailty, working closely with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support.

You will need to possess the necessary skills to deliver services to patients with a wide range of complex clinical needs, in particular acute presentations. Experience of working with patients suffering long term conditions would also be advantageous. As team leader you will also be supported with time allocated to non-clinical activities for the purpose of both team and service development. The abilities to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, as is working experience/knowledge of local pathways and referral procedures.

The role of Digital Navigator presents the successful applicant a pioneering opportunity with our innovative Frailty team. Since its inception, Jean Bishop Community Frailty Service have been at the forefront of providing anticipatory, patient-tailored care to frail residents of Hull and the surrounding areas.

Recognised nationally, our Centre continues to receive outstanding patient feedback and has been featured in key national healthcare publications and media.

This is the first role of its kind, embedded within a clinical team, to support both the clinical and administrative functions of the Frailty team.

This is an exciting opportunity for a technology-savvy individual who is passionate about improving care for older and frail patients. You will play a crucial role in deploying digital solutions that enhance patient care and operational efficiency.

You will need to possess strong interpersonal skills, a friendly patient-focused attitude, and the ability to put the patient first at all times.

Are you passionate about behaviour change? We have an exciting role available at Smokefree Liverpool as a community development worker to help achieve our commitment to reduce the prevalence of smoking and tackling health inequalities for the people of Liverpool. We are offering a flexible and varied role, training, working as part of a dynamic team, hybrid working and community-based interventions and prevention. Main duties of the job We are looking for an enthusiastic and motivated person to join our team in Liverpool. The successful candidate will deliver stop smoking advice, complete referral to service and target vulnerable populations within the city.

Providing very brief advise to smokers across a variety of setting the post holder will support referral into service, this will be done via promotional events, GP surgeries and other community venues. If you have good people skills the ability to support and motivate others and work well in a team setting, we would like to hear from you.
