Job Purpose The post holder will deliver Speech and Language Therapy care in a range of community environments to adults over 18 who are presenting with communication and swallowing difficulties. The team is unidisciplinary, but s/he will work with a range of other professionals, including physiotherapists, occupational therapists, psychologists, dieticians, nurses and medical consultants, social workers and a range of other carers and community personnel. S/he will encounter a wide range of physically and emotionally challenging conditions in clients homes and other environments in the community. Clinical Responsibilities To manage an autonomous caseload for the Neurorehabilitation service on Frank Cooksey Rehabilitation Unit and Ontario ward, which will include assessment, differential diagnosis, and specialist SLT management for patients with acquired communication and swallowing disorders, following King's College Hospital (KCH) Speech and Language Therapy evidence based guidelines and abiding by KCH Speech and Language Therapy service standards. To provide highly specialist input into policies and protocols regarding the management of patients seen by the Speech and Language Therapy Neurorehabilitation team and wider service. To act as a link between all SLTs involved in treating patients in Neurorehabilitation beds and jointly lead the team in order to ensure the patient receives treatment from the most appropriate therapist and that knowledge and resources are shared across the service. To work closely and in collaboration with the multidisciplinary team in Neurorehabilitation in assessment, the planning and provision of rehabilitation goals and programmes, negotiating length of stay and discharge arrangements as appropriate. To co-ordinate and promote communication developments under the guidance of a senior therapist e.g. mental capacity act issues, developing and delivering training on supporting communication, advising on environmental changes that support those with communication difficulties. To act as Rehabilitation Coordinator as appropriate. To provide complex and sensitive clinical information to patients and carers in clear, accessible and empathetic format, both in written and verbal form. To contribute to instrumental assessments (VF/FEES) and assist in the training and supervision of other SLT staff in these assessments as appropriate. To introduce low and high tech communication aids as appropriate and train the patient, their family and friends and the staff on using alternative and augmentative communication aid systems or to refer on to the communication aid therapist at Bowley Close. The Local Recovery Coordinator (LRC) functions as a champion and advocate for the recovery model. LRC also serves as a recovery ombudsman to the Associate Chief of Staff of Mental Health (ACOS-MH), program managers, and staff on recovery and implementation of recovery-oriented services. In conjunction with the ACOS-MH, the LRC develops a 3-5 year plan for the facility that identifies staged implementation of recovery-oriented services (e.g., use of recovery plans). LRC is responsible for coordinating the evaluation of recovery implementation at the facility and reporting results to the National Recovery Coordinator (NRC). With guidance from the NRC, the LRC will implement measures to assess recovery policy and procedure adherence, as well as assess the extent that the recovery model is embodied in the Mental Health programs. Some of the duties include, but are not limited to;

• Develops a facility recovery education plan for providers and Veterans and their families.
• Develops an ongoing education plan for providers, Veterans, and families that includes recurrent didactic presentations about elements of recovery (e.g., recovery plans) and recovery implementation by experts.
• Serves as consultant to Mental Health programs and recovering Veterans in development of peer support groups, consumer counseling, and consumer operated services, as per local needs and resources.
• Advises the Mental Health Program Managers on filling peer specialist positions and serves as a consultant to peers specialists.
• Assists with VA regional or local recovery-oriented trainings, meetings, or conferences as needed.
• Serves as a Mental Health Liaison to the facility Consumer Council.
• Provides an annual report on implementation progress to the VISN, RC and the NRC; other reports will also be prepared as requested.
• Meets quarterly via conference call with the VISN RC to coordinate a network response to recovery implementation and implementation of CO policies and procedures related to recovery.
• Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the Mental Health Service and/or their families.

Highly specialist SLT in critical and acute care managing complex communication and swallowing impairments including patients who are tracheostomised and ventilated.

Senior leadership and management of the acute SLT team.

- The post provides the main clinical cover for general and cardio thoracic critical care units and input across the hospital wards including medcard and front door services.

- Thepost is required to input into the multidisciplinary management of complex critically ill patients with communication and swallowing impairments, including those with tracheostomies and ventilatory requirements, as per the Intensive Care Society guidelines, these patients should receive daily SLT input therefore there needs to be cover across the week.

- The post holder’s expertise and input helps to support patient recovery and rehabilitation which in turn helps reduce secondary complications and ongoing care needs, aiding flow through the hospital and discharge.

The post holder provides senior support to junior staff including supervision, clinical support and caseload management across the majority of the adult inpatient wards.
