The Urgent Care Access hub and Home Treatment Team in Coventry is an Age independent service, One team with two functions, We operate a 24 hours service and the shift patterns are 11.5 hours. This fast paced, dynamic environment requires excellent problem-solving skills, the ability to make decisions under pressure and provide high quality care to people with mental health illnesses and social inequalities in their own homes. You will also be expected to Triage Urgent referrals which include Safeguarding and make decisions about care and treatment in a responsive manner, signposting to 3rdsector services and referring into secondary care services if needed. Giving current NICE guideline advice around self-help and treatment options via telephone triage. You will be supported by a senior leadership team of experienced nurses and an MDT of colleagues who are passionate about treating patients in their own homes in order to avoid hospital admission.
